Freedom Federal Credit Union Youth Month 2016 – Kids Art Contest Instructions and Guidelines 1. Original artwork only may be submitted. Artists must be in the age category under which they submit their work. That is: a. 4 to 7 b. 8 to 12 c. 13 to 17 2. The prize for calendar artist chosen for the 8-12 and 13-17 age categories will be a Kindle. 9. A gift receipt/1099-INT tax form must be signed by a parent or guardian when prize is claimed. 10. A photo release form may also be signed if each winner agrees to allow Freedom to publish a photo of the winner. 11.
