The Efficacy of Tranexamic Acid for Reducing Blood Transfusion Rates in Extracapsular Hip Fractures: A Single-Center Randomized Controlled Trial

Prospective randomized controlled trial evaluated whether tranexamic acid (TXA) given at hospital admission reduces transfusion rates and blood loss in extracapsular hip fractures. Patients with closed intertrochanteric or subtrochanteric femur fractures undergoing intramedullary nailing (n=100) received 1 g IV TXA or saline. Postoperative transfusion rates were lower with TXA (17.5% vs 36.7%), and total blood loss decreased (mean difference 367 mL, P=.01). No significant differences were found for intraoperative blood loss, length of stay, 30-day mortality, or major complications. Single-dose IV TXA at admission is recommended.
