This document provides a template for QAPI Committee Meeting Minutes, designed to help facilities effectively document their Quality Assurance and Performance Improvement meetings. It includes critical instructions for users to review, remove disclaimers, delete explanatory comments, and personalize or remove highlighted text before use. The template emphasizes that it is a guide and must be adapted to reflect an organization's unique priorities, noting it does not aim to meet specific regulatory requirements. A best practice suggestion is to assign a consistent note-taker who is distinct from the meeting facilitator. The document also contains a legal disclaimer regarding its protected nature, intended for review and evaluation by the QAPI Committee, and subject to peer review and other legal privileges, with restrictions on copying or distribution without written consent from the legal department. Ultimately, this template serves as a foundational document for documenting quality assurance and performance improvement initiatives within a facility.
