This document serves as the official Parent or Guardian Consent Form for the New Jersey Motor Vehicle Commission (NJMVC). It is required for all applicants under the age of 18 seeking permit or license transactions, and for applicants under 17 seeking a non-driver identification card. The form facilitates the collection of essential applicant data including name, date of birth, and residency address. Parents or legal guardians must provide their printed name, signature, and the date to legally authorize the minor's application. Additionally, the form functions as a verified Proof-of-Address document for the requested transaction. Users must accurately categorize the transaction type, such as standard licenses, Real ID, or specific endorsements like motorcycle or moped, ensuring all legal requirements mandated by N.J.S.A. 39:13-3 are met during the submission process. This structured template ensures that all necessary consent and identification information is captured clearly and efficiently for administrative processing.
