Milk substitution policy defining when cow’s milk can be replaced and what documentation is required. If requested in place of cow’s milk, a note must specify the exact substitute the child must drink, with two pathways. Parents’ notes apply only to soy milk from an approved federal brand list or lactose-free milk with specified age-related fat content. Any other substitute requires a completed physician-approved Medical Statement, confirming cow’s milk sensitivity, omission, and the specific substitute; incomplete statements are not approved and prevent meal counting.

FAQs

When is a note required for a milk substitution request?

A note is required whenever milk substitutions are requested instead of cow’s milk, and it must specify what milk substitute the child must drink.

What substitutions qualify for a Parent’s Note?

A Parent’s Note is sufficient only for soy milk from the 5 federally approved soy brands list or lactose-free milk, with lactose-free milk requirements based on the specified age group.

What documentation is needed for substitutions other than soy or lactose-free milk?

A Doctor’s Note is required using the approved Medical Statement for Children with Disabilities Form, completed by a qualified physician, PA, or ARNP, and it must confirm sensitivity to cow’s milk, cow’s milk omission, and the exact substitute.

What happens if the medical statement is incomplete?

If the Medical Statement does not include all required elements, it will not be approved, and the child’s meal cannot be counted.
