Sep 17, 2026

This document provides a standardized template for a Homeowners Association (HOA) General Proxy Form, designed to facilitate the appointment of a proxy holder for association meetings. The form captures essential meeting details, including the association name, date, time, and location, alongside member information such as the owner's name and property unit address. It allows members to legally appoint a representative to vote on their behalf, with optional sections provided for recording specific limitations or special voting instructions. This template ensures that all necessary legal disclosures and signature requirements are met to maintain valid voting proceedings within an association's governance framework. It is an essential administrative tool for HOA boards and members to ensure effective quorum and representation at official meetings, particularly when direct attendance is not possible for all members.
