Per the ACA, the State of Iowa is required to provide verification of health insurance coverage for all employees who worked an average of 30 hours or more per week and were offered health insurance from the State during the calendar year.

Form 1095-C is provided as proof of qualifying health insurance coverage the State of Iowa offers you and your family. It contains information about who provides your health insurance, which members of your family are covered (if applicable) and the months of the year each person was covered.
Please keep Form 1095-C for your records. It contains information that will be necessary for completing your Federal income tax return.
Only one form is provided for all covered individuals. You may need to provide copies to your dependents.
As required by ACA, the information provided on Form 1095-C will also be reported to the IRS. | Q3: What specific information is included on Form 1095-C?
