The template facilitates a comprehensive administrative record by documenting the student's identification details, the specific nature of the violation, and the assigned disciplinary sanctions ranging from verbal warnings to expulsion. It further provides a structured area to capture a factual summary of the incident and a historical record of prior disciplinary actions to ensure consistency in student conduct enforcement. Additionally, the form explicitly outlines the student's rights regarding the appeals and grievance process as established in the Mercy College Catalog, while serving as a formal acknowledgment of receipt through a mandatory signature section.
