This document provides an Action-Plan Template for the implementation of Standards-Based Grading (SBG) on a schoolwide or districtwide basis. It aims to guide teams in assessing their current level of implementation on a continuum from 'Getting Started' to 'Implementing' and 'Sustaining'. The template features a table designed for strategic planning, where teams can define their desired level of achievement and specific criteria. It prompts users to detail the necessary actions to progress towards sustaining SBG, identify what needs to be changed or relinquished to facilitate this progress, and specify the evidence, such as products, policies, or procedures, that would demonstrate the achievement of their desired criteria. The document also includes a space for teams to indicate their current level of implementation by circling one of the three options: 'Getting Started', 'Implementing', or 'Sustaining', and providing supporting evidence. This resource is intended to facilitate a systematic and evidence-based approach to adopting and embedding standards-based grading practices across educational institutions.
