Speakers: Samuel Klein, Weight Management Program, Washington University Physicians Robert Whitaker, Temple University Michelle Cardel, University of Florida Agenda Topic: Overview to understanding obesity (Klein) • Metabolically normal vs. abnormal obesity • Obesity caused by imbalance of energy intake and energy expenditure • We need to focus on the whole patient o Too much focus on BMI and body weight • The importance of diet composition – the Mediterranean diet • Food commercials and time spent watching TV are critical environmental factors • The biggest gains in BMI occurred after WW II. Agenda Topic: Biopsychosocial risk factors for childhood obesity (Whitaker) • Focus is on the role of self-regulation in preventing obesity among children • Supporting children’s self-regulation might allow for the disruption of intergenerational transmission of obesity and other health inequalities. • What is self-regulation? Process by which we manage our thoughts and feelings in order to (a) alter the intensity, frequency, and duration of what we say or do in social settings; (b) start and stop activities to meet the demands of a particular situation; (c) delay acting upon a desired goal; (d) act in a socially approved manner even when no one is watching. o Three interrelated domains: Emotional, cognitive, and behavioral. These domains are often tightly integrated. o In children, emotional self-regulation develops before cognitive self-regulation. • How is self-regulation relevant to obesity? o Poor self-regulation is associated with the development of a higher BMI and a greater risk of being overweight. o Poor self-regulation is associated with outcomes that are prioritized more than the prevention of obesity – for example, doing well in school; making friends. o Poor self-regulation may also be a mechanism by which socioeconomic disadvantage during childhood contributes to obesity and the prevention of other outcomes that are prioritized by parents. o Self-regulation can be altered by experience; there are some things that can be done to support the development of the capacity of children to self-regulate. o How to support the development of self-regulation in children? 􀂃 Positive parenting helps create an environment of perceived safety. 􀂃 Positive parenting leads to secure attachment. 􀂃 Does a daily routine for young children enhance self-regulation and prevent obesity? Agenda Topic: Social Determinants of Obesity (Cardel) • Research literature on obesity indicates two common themes: o Focus on individual characteristics oflow SES o Focus on the obesogenic/built environment in low SES neighborhoods • Perceiving oneself to be in a lower social status position leads you to consume more calories because your environment does not feel secure. Animal studies show this. Examples in humans: o If your sports team loses, you will consume excess calories (loss by sports team leads to feeling of lower social status) o Moving to a higher income neighborhood (a more secure environment) predicted a lower prevalence of obesity 10-15 years later. o What is the mechanism? Can manipulate social status in the lab – for example, even losing a Monopoly game leads to greater caloric consumption. o Conclusion is that being in a low social status position may influence eating behavior and risk for obesity.
