This clinical reference guide provides a comprehensive overview of ST-Elevation Myocardial Infarction (STEMI) location diagnostics based on 12-lead electrocardiogram changes and associated arterial supply. It details the PAILS mnemonic—Posterior, Anterior, Inferior, Lateral, and Septal—to assist healthcare professionals in identifying infarct sites. The document explains the physiological significance of lead changes, the concept of reciprocal changes on the opposite side of the myocardium, and critical coronary artery involvement, such as the LAD 'widow maker' and inferior wall 'boot' patterns. By standardizing the interpretation of ST-segment elevation and depression, this resource serves as an essential tool for rapid cardiac event assessment and clinical decision-making. The included summary table and annotated ECG trace facilitate accurate localization of myocardial injury and help clinicians correlate electrical patterns with specific coronary anatomical structures effectively.

FAQs

What does the PAILS mnemonic represent in the context of STEMI?

PAILS is a memory aid for identifying STEMI locations: Posterior, Anterior, Inferior, Lateral, and Septal wall infarctions.

Why is the anterior wall infarction often referred to as the 'Widow Maker'?

It involves the Left Anterior Descending (LAD) artery, which is the largest coronary artery; its blockage affects a significant portion of the heart's myocardium.

How do reciprocal changes assist in confirming a STEMI?

Reciprocal changes are ST-segment depressions observed in leads opposite to the site of the infarction, providing diagnostic confirmation of a true STEMI event.
