Severe early onset scoliosis (EOS) requires surgery but is difficult due to limited fixation options, very large curves, and vulnerable patients with comorbidities. Magnetically controlled growing rods (MCGR) can avoid routine operative lengthening, yet effectiveness for severe EOS was unclear. A retrospective matched analysis of 44 children (≥90°) compared MCGR with traditional growing rods (TGR) at 2 years using EOSQ-24 and curve outcomes. MCGR achieved greater major curve correction and higher unplanned revision-free survival with no quality-of-life differences.
