This document outlines the American Heart Association (AHA) Advanced Cardiovascular Life Support (ACLS) algorithm for managing patients following the return of spontaneous circulation (ROSC). It provides critical clinical guidance for healthcare professionals regarding 12-lead ECG assessment, emergent cardiac interventions for STEMI or unstable cardiogenic shock, and precise ventilation/oxygenation protocols including target PETCO2 and oxygen saturation levels. Furthermore, it details essential IV bolus and medication management, featuring specific infusion rates for Epinephrine, Dopamine, and Norepinephrine. Designed as a rapid-reference clinical tool, this algorithm emphasizes evidence-based post-resuscitation care to optimize patient outcomes. The structured flow serves to standardize clinical decision-making during the high-stakes period immediately following cardiac arrest recovery, ensuring consistent application of life-saving measures across medical settings.

FAQs

What are the primary criteria for considering emergent cardiac intervention?

Emergent intervention should be considered if the patient exhibits a STEMI, shows signs of unstable cardiogenic shock, or requires mechanical circulatory support.

What are the target parameters for ventilation and oxygenation in post-ROSC care?

Care providers should aim for a ventilation rate of 10 breaths per minute, titrate to a PETCO2 of 35-45 mmHg, and maintain oxygen saturation between 92% and 98%.

Which medications are indicated for hemodynamic support in the algorithm?

The protocol recommends utilizing an IV bolus (NS or LR) of 1-2 liters as needed, alongside infusions of Epinephrine (2-10 mcg/min), Dopamine (5-20 mcg/kg/min), or Norepinephrine (0.1-0.5 mcg/kg/min).
