It began in a Detroit hospital when apatient awaiting an eye operation devel-oped signs of heart failure. For 12 hours they fought to save hislife. And during those 12 hours,he wasattended by 15 physicians,as well as bynumerous nurses and technicians. Despite every resource of medical sci-ence,the patient died. But it was not be-cause of lack of expert, devoted care.In-deed those 12 hours testified to the heroicefforts of the health professionals to saveand prolong a human life. The HonorableEDMUND G.BROWN,JR. Governor of California *If all tickets are sold out, only those with paid luncheon tickets willbe admitted into the room. It was from another point of view,how-ever,that the case was presented to theAmerican people. Testifying before a Senate Subcom-mittee investigating health insurance,Leonard Woodcock, the President of theUnited Auto Workers, called the cost ofthe treatment(almost $8,000)‘scandalous.'A newspaper headline writer put itthis way:'Modern Medical Price:Deathand Brankruptcy for One Man's Family.' Clearly, what the doctors involved hadseen as a valiant attempt to save a life hadbeen transmuted into another indictmentagainst the American medical system. Indeed,at a recent conference on healthcare costs,an observer pointed out thatthe phrase 'cost effective'was referred to(Continued en next page) GRAND BALLRO0M,FAIRMONT HOTEL "MANY BELIEVE WE MUST FACE THE MORAL AND THELEGAL ISSUES INHERENT IN COST-BENEFIT ANALYSIS” 71一115 FROEHLKE(Continued from preceding page) at least 50 times, but the term ‘quality ofcare'was not heard once. The medical ethic up to now has com-pletely rejected any cost-benefit analysis. Many believe that we must face up to themoral and legal issues inherent in cost-benefit analysis of the practice of medi-cine before any substantial inroads willbe made in the inflationary spiral of healthcare costs. ## Cost-Benefit Analysis Employers are finding that insurancecarriers are becoming their largest ‘sup-plier³of services. The most widely pub-licized case is General Motors.GM dis-covered last year that the cost of healthcare adds more to the price of its carsthan the cost of steel. You don't have to be a General Motors, however,to know what's happening.Allemployers are feeling the pinch as theirhealth insurance outlays go up and upThey're going up for the obvious reasons:health care costs, led by hospital roomrates, are rising faster than all majorgoods and services in our economy. AP 11 Why this is happening is the subject offurious debate.Today the economistsseem to be saying that the most powerfulinfluences on costs are as follows: One,hospital care is a labor intensiveoperation. As labor costs inflate, health 77 care costs will be compounded. Two,technological advances in medi-cine have been phenomenal in the lastgeneration. They all have been a boon tolongevity. They also have added immeas-urably to the cost of facilities, trainingand personnel. Three, whatever solution is finally developed will have to involve the coopera-tion of the doctor and patient. To date, noeconomic incentive has come to lightwhich rewards either the doctor or thepatient for economical medical care. ## Mix Quality and Price Doctors and patients alike have simplynot had the incentive to seek the rightmix of quality and price. The tendencyis to go for the most costly care, andwhy not—goes the reasoning—the insur-ance pays for it. Four,is the fear of a malpractice suit.Huge awards in malpractice suits have leddoctors to practice what's come to becalled 'defensive medicine'—t
