Nursing homes frequently struggle to address residents’ goals, preferences, and priorities (GPPs), with regulatory F-tag F656—Comprehensive Care Plans—among the most cited deficiencies in surveys. A practical, user-friendly guide was created to help staff elicit what matters most to residents and integrate those inputs into care plans and daily care. The Kansas State University Center on Aging is piloting the approach in up to 30 nursing homes with education, individualized quality improvement action planning, and implementation evaluation. Pilot findings will support revisions and inform broader systems change. The work also offers preliminary lessons for health information technology and for measures supporting goal-concordance monitoring relevant to federal quality oversight.
