2026 Biometric Screening and Clinical Health Risk Assessment Physician Lab Form Help Guide To save $500 in 202 7, complete your lab work, get the Physician Lab Form completed and signed, upload it to UMR’s Live Well Reward$ platform, and complete the clinical health risk assessment (CHRA) on Live Well Reward$ between January 1 and November 30, 2026. Activities completed outside of that time period do not qualify. Processing of the Physician Lab Form is manual. We strongly recommend using the Direct-to-Quest option or an onsite biometric screening instead. If you choose to use the Physician Lab Form, please upload it to Live Well Reward$, rather than faxing it, and upload it as early as possible to allow for the longer processing time. Use the Physician Lab Form if you prefer to: • Get lab work that’s ordered by your doctor which may include additional tests not required for a biometric screening (such as thyroid) . • Use an in-network lab other than Quest (this requires your doctor to order the labs) . • Go to a convenience care clinic such as CVS or Walgreens. Biometric Screening (Physician Lab Form via UMR) 1. Log [into](into UMR.com)[ ](into UMR.com)[UMR.com](into UMR.com) (if needed, see UMR Login for step-by-step instructions). 2. In the blue menu at the top, click on Health Center, then Live Well Reward$ under Wellness Programs. 3. Under Live Well Reward$, click Get Started. 4. In the Live Well Reward$ Wellness Platform, in the middle column, scroll down to the Physician Lab Form section. 5. Click the Download button to download and print your personalized form. See a sample form at right. Review the downloaded form to verify that your personalized information is accurate. NOTE: The A1c test, which measures your average blood sugar level, is optional but available to all employees, with or without a diabetes diagnosis. 6. Complete your screening and upload your form prior to the November 30 deadline using one of the options below. a. Option 1-In-network lab such as BayCare or Labcorp: • Have your doctor order the lab work. Make sure it includes all required items (A1c is optional) . • Schedule an appointment at the lab of your choice. Do not use the Direct-to-Quest Patient Service Center appointment scheduler if using this method. It will generate an additional lab order that may cause confusion and problems later. • When you receive the results, ask your doctor to complete and sign the form. b. Option 2-Convenience care clinic such as Walgreens or CVS: • Ask the pharmacy staff who does the screening to complete and sign the form. SAMPLE 7. Upload your form: a. Sign and date the completed form. b. Upload the form electronically on Live Well Reward$ in the Physician Lab Form section. • Click Upload. • Click Choose file and navigate to the file which can be a PDF if you scan it or an image file if you take a photo of it (jpg or png) . Click Upload. c. Processing time is about 1 week for an uploaded form and 3 weeks for a faxed form due to manual processing. It is highly recommended that you upload the form instead of faxing. 8. Confirmation (Live Well Reward$ Platform) : a. Message: Click the envelope with a red dot at the top right of the screen to access a confirmation message that your Physician Lab Form has been received. The message will alert you if any lab results are missing or illegible. If the message says Missing Biometrics: None, your form is approved. b. Checkmark: In the Track Your CHRA and Biometric Screening Progress section, click More to see a green checkmark next to Biometric Screening Completion status. c. Upload Verification: If you upload the form (rather than faxing), you will see Show Uploaded Documents in the Physician Lab Form section instead of No Uploaded Documents. Need help?
