An automated email from GoToWebinar will also be sent to all attendees that will include a link to the survey/evaluation. \nPlease note: the certificate that will be linked in GoToWebinar’s automated “thank you for attending” email is not your CEU certificate. Your official certificate will be sent out by QIPMO staff within 1-2 weeks of the webinar. \nW \nRI TING \nA \nP \nLAN \nOF \nC \nOR RECT ION \nN I C KY MA RT I N , M PA , L N HAQ I P M O L EA D E R S H I P C OAC H \n• Identify the required elements of a plan of correction (POC) \n• Learn tips on: \n– Planning \n–writing \n– implementing, and \n– monitoring a POC. \nPLANNING \nWHAT IS A POC? \n42 CFR §488.401 defines a Plan of Correction to mean a plan developed by the facility and approved by CMS or the survey agency that describes the actions the facility will take to correct deficiencies and specifies the date by which those deficiencies will be corrected. \nWHY A PLAN OF CORRECTION ( POC) ? \n• To encourage facilities to correct deficiencies as soon as possible. \n• Commitment to correct each deficiency by a specific completion date. \n• Allegation of compliance as of the latest POC correction date given in the POC \n– If you use several dates, the latest date is automatically used \n• Submission of an acceptable POC is required for all deficiencies of scope and severity Levels B through L. \n• Must be submitted to state within 10 calendar days of receiving the 2567 \nPLANNING FOR YOUR POC \nWriting a plan of correction should begin during the survey.Taking notes throughout the survey process and during the exit will be especially important. And remember, a plan of correction is… \n– A legal document \n– A formal statement informing state and federal agency of actions taken to correct deficiency \n– Publicly posted-use CMS QCOR to see which other homes in your region have received the same deficiencies. \nSOM CHAPTER 7 \nIf the plan of correction is unacceptable for any reason, the State will notify the facility in writing. If the plan of correction is acceptable, the State will notify the facility by phone, e-mail, etc. Facilities should be cautioned that they are ultimately accountable for their own compliance, and that responsibility isnot alleviated in cases where notification about the acceptability of their plan of correction is not made timely. \nThe plan of correction serves as the facility’s allegation of compliance and, without it, CMS and/or the State have no basis on which to verify compliance.A plan of correction must be submitted within 10 calendar days from the date the facility receives its Form CMS-2567. If an acceptable plan of correction is not received within this timeframe, the State notifies the facility that it is recommending to the RO and/or the State Medicaid Agency that remedies be imposed effective when notice requirements are met.The requirement for a plan of correction is in 42 CFR 488.402(d). Further, 42 CFR 488.456(b)(ii) requires CMS or the State to terminate the provider agreement of a facility that does not submit an acceptable plan of correction. \n• Collecting notes \n• Copying of documents \n• Sharing information between team members \n• Identifying potential concerns \n• Monitoring for trends \n• Create a binder \n• Begins upon exit \n• Consider staff assignments \nP LA N N I NG FO RYO U R PO C \nWRITING \nFIVE ELEMENTS OF A POC \n• Element 1 \n– Address how corrective action will be accomplished for those residents found to have been affected by the deficient practice \n• Element 2 \n– Address how the facility will identify other residents having the potential to be affected by the same deficient practice \n• Element 3 \n– Address what measures will be put into place or systemic changes made to ensure that the deficient practice ","cbCaic4OIiJIkJQc","https://ap.wps.com/l/cbCaic4OIiJIkJQc","pdf",1037521,19,"English","# Planning\n## What is a POC?\n## Why a Plan of Correction (POC)?\n## Planning for Your POC\n# Writing\n## Five Elements of a POC\n## What You Need to Do\n## How You Can Do It","[{\"question\":\"What is the definition of a Plan of Correction (POC)?\",\"answer\":\"A POC is a plan developed by the facility and approved by CMS or the survey agency, describing actions to correct deficiencies and the dates by which those deficiencies will be corrected (42 CFR §488.401).\"},{\"question\":\"When must a facility submit its POC after receiving Form CMS-2567?\",\"answer\":\"A plan of correction must be submitted within 10 calendar days from the date the facility receives its Form CMS-2567.\"},{\"question\":\"What are the five elements that must be included in a POC?\",\"answer\":\"The POC must address how corrective action will be accomplished for affected residents, how other potentially affected residents will be identified, what systemic changes will prevent recurrence, how performance will be monitored to ensure solutions are lasting, and the completion dates for corrective action.\"}]","Writing a Plan of Correction (QIPMO) - Webinar CE | PDF",1790139110,7]
