It supports change leaders by adding dimensions to existing strategies, helping teams consider critical dimensions and adapt to context. The model is not prescriptive and can be applied to any scale of change, from small initiatives to system-wide transformation. It highlights eight interconnected components spanning purpose, spread, improvement tools, project and performance management, measurement, system drivers, motivation, and Model components \nPrinciples that underpin the Change Model \nAcknowledgements \nIntroduction \nGreat change needs strong foundations. Making change happen isn’t easy, but we hope this guide will help. \nThe Change Model provides a valuable framework to enable effective and sustainable change that delivers real benefits for staff, patients and communities. It has been co-produced with hundreds of health and care staff and based on credible evidence and experience. The Change Model is for anyone who wants to make a difference – you could be in a clinical or a support role at any level of an organisation. \nWe can use the model with any change that matters tous, no matter how big or small. It could be within: \n• Our own work \n• A department \n• An organisation \n• Across a whole system. \nEach project will be slightly different so the model isn’t prescriptive. It should help you to consider what matters most in your particular setting and achieve a balanced approach. \nThe Change Model is an integral, interconnected framework that works in harmony with the model for large scale change to support sustainable transformation. However, it is important to note that the Change Model works for any scale of change, even a small one. \nLeadership by all \nSpread and adoption \nMotivate and mobilise \nSystem drivers \nOur shared purpose \nImprovement tools \nProject and performance \nmanagement \nMeasurement \nFigure 1: The Change Model \n 3 \nThe Change Model Guide \nIntroduction \nUsing the \nChange Model \nChange Model components \nPrinciples that underpin the Change Model \nAcknowledgements \nThe Change Model components \nThe Change Model has eight components which set out the elements that should be considered when planning and implementing change. These provide an organised, systematic approach to help create an environment for successful change. This document is a guide through the principles that underpin the Change Model and each of the eight components. \nThe eight components: \n• Our shared purpose-the starting point \n• Spread and adoption \n• Improvement tools \n• Project and performance management \n• Measurement \n• System drivers \n• Motivate and mobilise \n• Leadership by all \nMost importantly the Change Model is a framework not a methodology. The Change Model provides us with ideas, prompts, tools, and resources that you can use foryour own unique situation: it provides a systematic way to consider the critical dimensions that might affect your change programme. It is important to recognise that some of the components may not be within your control on a project but it is always worth while being informed about them and the role that they play. \nThe Change Model can be used for any change initiative, big or small, and in any organisation, and for any kind of change you are trying to create. \n 4 \nThe Change Model Guide \nIntroduction \nUsing the \nChange Model \nChange Model components \nPrinciples that underpin the Change Model \nAcknowledgements \nThe Change Model support","cbCaimxeZcZ893DP","https://ap.wps.com/l/cbCaimxeZcZ893DP","pdf",2392533,32,"English","# Introduction\n## Purpose and scope of the Change Model\n# Using the Change Model\n## Change leaders and acceleration of effective change\n## Applying the model at any scale\n# Change Model components\n## Our shared purpose-the starting point\n## Spread and adoption\n## Improvement tools\n## Project and performance management\n## Measurement\n## System drivers\n## Motivate and mobilise\n## Leadership by all\n# Figure 1: The Change Model\n# Organising framework for large scale change\n## Navigation of complex system-wide change\n# Approaches, Methods & Tools\n## Recognised methodologies and improvement tools","[{\"question\":\"What is the Change Model Guide and who is it for?\",\"answer\":\"The guide presents a framework for anyone seeking to make meaningful change in health and care, including clinical and support roles at different organisational levels.\"},{\"question\":\"How can the Change Model be used across different sizes of change?\",\"answer\":\"The model works for any scale—from small initiatives to large, system-wide change—and is adaptable rather than prescriptive.\"},{\"question\":\"What are the eight components of the Change Model?\",\"answer\":\"The eight components are our shared purpose, spread and adoption, improvement tools, project and performance management, measurement, system drivers, motivate and mobilise, and leadership by all.\"}]","The Change Model Guide - Using the Change Model | PDF",1789158192]
