this office has substantiated that New York City Department ofEducation(\"DOE”)Paraprofessional Andrea Sirico(\"Sirico\"),assigned to Public School 369(\"P.S.369\")in Brooklyn, submitted falsified documents and failed to cooperate with investigators of the Office of theeSpecial Commissioner of Investigation for the New York City School District(“SCI”). \n## Investigation &Findings:\n\n### a.Initial Complaint:\n\nThe investigation began in December 2024,when SCI received a complaint from, who reported that \nreceived a request from Sirico for paidleave because of a death in the family(a\"DIF Request\").In processing the DIF Request, andnoticed that Sirico had requested a suspiciously large number of such leave days.Upon reviewof Sirico's file, and informedthat Sirico had submitted DIFRequests supposedly related to the deaths of 18 different relatives since 2015.²To support her latest DIF \nRequest,Sirico had provided P.S.369 a letter purporting to be from the Cobble Hill Chapels funeral homein Brooklyn, documenting her attendance at funeral services allegedly provided there (a\"FuneralLetter”).³Given the suspicious volume of Sirico's DIF Requests, contacted CobbleHill Chapels and spoke to a funeral director who informed her that he had not provided funeral servicesfor the individual listed on Sirico's Funeral Letter. \n### b.Sirico's Funeral Letters:\n\nUpon request, andprovided SCI with all 21 Funeral Letters in Sirico'sfile. All Funeral Letters looked to be on letterhead belonging to various funeral homes in Brooklyn,with16 of the Funeral Letters purporting to come from Cobble Hill Chapels.According to the Funeral Letters, since May 2016,Sirico had suffered the deaths of at least:(1)two grandfathers;(2)seven uncles;(3)fiveaunts;⁴(4)two cousins;(5)one fiancé;(6)one father-in-law;and(7)two individuals where therelationship to Sirico could not be determined (either because no relationship was listed or because theFuneral Letter was illegible).Pertinent details of Sirico's Funeral Letters include: \n\n| Name \u003Cbr>of \u003Cbr>Deceased⁵ | Relationship to Sirico |
