Three common types of coverage requests are coverage determination, prior authorization, and medical exception/appeal. A benefits verification may help identify the specific process and documentation required by the plan. \nAn LMN is a critical component of appealing a coverage denial or requesting a formulary exception. The LMN is the prescribing physician’s opportunity to present all of the rationale for why a certain ViiV Healthcare product is clinically necessary for the patient. It is important to note that LMNs should be concise while making as strong an argument as possible and should be drafted on official letterhead stationery. \nConsider including the following items in an LMN: \n• Clearly state the rationale for therapy with the relevant ViiV Healthcare product and why it is appropriate for the patient \n• Including support for the therapy recommendation, citing published trials and DHHS guidelines, can be impactful \n• Specify if the patient is already on the ViiV Healthcare product and is clinically stable, and include specific measures of clinical benefit \n• Explain why the formulary-preferred agents, if applicable, are not appropriate \n• Outline implications if patient goes without therapy or any issues that you think may arise if the patient is required to switch medications \nThe following is a template letter of medical necessity that you can customize based on your patient’s medical history and demographic information. Please note that some payers may have specific forms that must be completed in order to request prior authorization or to document medical necessity. \nADR 2024.12 ViiV Sample Letter of Medical Necessity \n1 This information is presented for informational purposes only and is not intended to provide reimbursement or legal advice.
