Jason Gray, J.C.D.\nTEMPLATES\nThe following are a series of templates that can serve as a point of reference for the juridical-canonical completion of the diocesan or eparchial phase of a Cause for Beatification and Canonization in preparation for the Roman phase before the Dicastery of the Causes of Saints.\nThese consist of templates that can be used by those who carry out a Cause, adapting them to the particular and unique circumstances of each case.\nAccording to the nature of the Cause, the following language in the templates must be adapted as follows:\nThe title and the lemma (definition) of the Cause\nWhen treating the martyrdom of a Servant of God: “Cause of Beatification or declaration of martyrdom of the Servant of God [name and title, including state of life in the church], allegedly killed in hatred of the faith”;\nWhen treating the cause of the martyrdom of several Servants of God: “Cause of Beatification or declaration of martyrdom of the Servants of God [name and title, including state of life in the Church of the head of the group] and his [her] X companions [precise number of the accompanying martyrs], allegedly killed in hatred of the faith”;\nFor the cause of a Servant of God who practiced heroic virtue: “Cause of Beatification and Canonization of the Servant of God [name and title, including state of life in the church]”;\nFor of a Servant of God who offered his or her life for another: “Cause of Beatification and Canonization of the Servant of God [name and title, including state of life in the church]”.\nThe doubt and the object of the Inquiry\nFor a Servant of God whose Cause is proposed according to martyrdom:  “on the life and the martyrdom as well as the reputation of martyrdom of the servant of God [name and title, including state of life in the church]”;\nFor the Servant of God whose Cause is proposed according to a life of heroic virtue: “on the life and the heroic virtue as well as the reputation of holiness and of signs of the Servant of God [name and title, including state of life in the church]”;\nFor the Servant of God whose Cause proceeds according to the offering of life: “on the offering of life until death out of charity as well as the other virtues in an ordinary grade and possibly on the reputation of holiness and of signs of the Servant of God [name and title, including state of life in the church]”;\nFor an alleged miraculous healing: “on the presumed miraculous healing of [name of the one healed], attributed to the intercession of the Servant of God [Venerable Servant of God] [Blessed] [name and title, including state of life in the church].”\nMandate to procure the postulator of a cause [on letterhead of the petitioner of the cause]\n[name of postulator]\n[address]\nTo [name and title of the postulator]\nI, the undersigned, [name of the authorized representative:  Bishop, Superior General, President of the Association, Committee, or Guild, etc.], as Actor of the Cause of Beatification and Canonization of the Servant of God N.
