A letter of medical necessity helps explain the physician’s rationale and clinical decision-making in choosing therapy for a specific patient and may include supporting documentation (eg, medical records, clinical treatment history, prescribing information, and peer-reviewed literature). The letter may be submitted as part of the prior authorization (PA) process, with the claim form, as part of an appeal, or in response to a health plan’s request for additional documentation. \nThis resource provides information on the process of drafting a letter of medical necessity. Included below is a checklist that can be followed when creating a letter of medical necessity. In addition, attached to this document is a sample letter that includes information health plans often require. \nPreparing an Effective Letter of Medical Necessity \n When requesting treatment for your patient, follow the patient’s plan requirements, which may require specific forms for documenting a letter of medical necessity; otherwise, treatment may be delayed \n Provide complete, comprehensive information regarding your patient’s condition and the clinical rationale for treatment. Information recommended for a letter of medical necessity typically includes: \n• Patient information \n– Full name \n– Date of birth \n– Case ID number (if available) \n– Insurance ID/group number \n– Diagnosis, including ICD-10 CM code(s) \n• Summary of previous treatments \n– Medication \n– Clinical outcomes \n– Treatment duration \n– Discontinuation rationale (if applicable) \n• Current condition and severity \n– Current symptoms \n– MGFA classification \n– MG-ADL and QMG score \n• Clinical rationale for treatment \n– Medical history \n– Physical examination \n– Trial data \n– Dosing and administration \n• Summary of your recommendations \n Attach documentation that supports your recommendations (as applicable): \n• Additional rationale for treatment • Additional patient information \n– Prescribing information – Patient medical records \n– Clinical trial data – Clinical notes \n– Peer-reviewed literature – Lab results \n– FDA approval letter \nINDICATION \nRYSTIGGO (rozanolixizumab-noli) is indicated for the treatment of generalized myasthenia gravis (gMG) in adult patients who are anti-acetylcholine receptor (AChR) or anti-muscle-specific tyrosine kinase (MuSK) antibody positive. \nIMPORTANT SAFETY INFORMATION \nRYSTIGGO is associated with important warnings and precautions, including increased risk of infection, drug-induced aseptic meningitis, and hypersensitivity reactions. The most common adverse reactions (≥10%) in patients with gMG are headache, infections, diarrhea, pyrexia, hypersensitivity reactions, and nausea. \n*Use of the information in this letter does not guarantee that the health plan will provide reimbursement for RYSTIGGO. The information in this letter is not intended tobe a substitute for, or an influence on, your independent medical judgment. It is presented for informational purposes only and is not intended to provide reimbursementor legal advice.
