This service sees patients that have been in the hospital for a long period of time and are on the verge of needing possibly residential home placements. Pathways are created to enable them to come home and decide with us (over a two week period) and social services whether things can work at home and allows them time to settle within their own environment.  \nAs a Registered Nurse, your role will be to assess the patient within the Hospital environment, co-ordinator pathways and communicate with all partners, including family members. Working with a team of Personal Nursing Assistants, direction will be provided to provide daily care with the view to reduce activity over the two week period.  \nHilton’s Registered Nurses work within a professional team to maximise a positive and supportive outcome to each patient safely and within the limitations of our regulative obligations.  \nTasks include:  \n• Undertaking assessments on the patient while in the Hospital environment  \n• Communicate to all internal and external partners  \n• Arrange all pathways prior to the patient being discharged  \n• Co-ordinate discharge  \n• Undertake an intense assessment over a 1 to 3-day period  \n• Support with patient’s rehabilitation on returning home from a stay in hospital  \n• Manage the team of Personal Nursing Assistants in following the agreed person-centred plan  \n• Provide professional leadership, advice and guidance to Hilton Nursing Partners  \n• Communicate effectively with patients and their relatives, ensuring accurate and seamless information is conveyed and documented  \n• Assistant with any medication requirements  \n• At the end of the 2 weeks period, after a final assessment provide a full recommendation of ongoing needs for Social Services  \n• To follow Hilton Partners business and system process at all times.
