It defines the plan’s purpose, roles under the command/HICS structure, and how essential recovery resources—such as patient medical records and critical IT applications—are acquired, accessed, and restored. It also outlines department overview requirements, mission-critical services, activation measures, and coordination steps. Annual exercises, training, and updates with emergency preparedness leadership ensure continuity targets tied to RTO and recovery strategies.","\u0003
\u0003Diagnostic Imaging Department\u0004
\u0003Illinois Critical Access Hospital Network\u0004
\u0003 \u0004
\u0003Business Continuity Plan Example\u0004
\u000f\u0004
COOP Plan 2017
Submitted by:
Your Name and position
Contact Information
Hospital Name
Address, City and State and Zip
Name of your CEO:
(You Can add a picture of your hospital here or your logo
You can also insert the Emergency Preparedness symbol here as well
\u000f
Table of Contents
\u0013 TOC \\o "1-2" \u0014Section I: General \u0013 PAGEREF _Toc296320663 \\h \u00144\u0015
Section II: Department Overview \u0013 PAGEREF _Toc296320664 \\h \u00145\u0015
Section III: Activation \u0013 PAGEREF _Toc296320665 \\h \u00146\u0015
Section IV: Department Requirements \u0013 PAGEREF _Toc296320666 \\h \u00147\u0015
Mission Critical Services and Processes \u0013 PAGEREF _Toc296320667 \\h \u00147\u0015
Interdependencies \u0013 PAGEREF _Toc296320668 \\h \u00148\u0015
Mission Critical Equipment and Supplies \u0013 PAGEREF _Toc296320669 \\h \u00149\u0015
Vendors/Resources Call List \u0013 PAGEREF _Toc296320670 \\h \u001410\u0015
Mission Critical IT Applications \u0013 PAGEREF _Toc296320671 \\h \u001411\u0015
IT and Communications Downtime Procedures \u0013 PAGEREF _Toc296320672 \\h \u001411\u0015
Mission Critical Vital Records \u0013 PAGEREF _Toc296320673 \\h \u001413\u0015
Personnel \u0013 PAGEREF _Toc296320674 \\h \u001414\u0015
Section V: Department Continuity and Recovery \u0013 PAGEREF _Toc296320675 \\h \u001416\u0015
Initial Actions \u0013 PAGEREF _Toc296320676 \\h \u001416\u0015
Loss of Corporate Services \u0013 PAGEREF _Toc296320677 \\h \u001417\u0015
Alternate Location \u0013 PAGEREF _Toc296320678 \\h \u001419\u0015
Recovery and Resumption of Mission Critical Services \u0013 PAGEREF _Toc296320679 \\h \u001423\u0015
Appendix A: Schedules \u0013 PAGEREF _Toc296320680 \\h \u001424\u0015
BCP Update Schedule \u0013 PAGEREF _Toc296320681 \\h \u001425\u0015
BCP Exercise and Training Schedule \u0013 PAGEREF _Toc296320682 \\h \u001426\u0015
\u0015
\u000f
Section I: General
The function of the Business Continuity Plan(BCP) is to assist impacted areas in ensuring that critical business functions are maintained, restored, or augmented to meet the designated Recovery Time Objective (RTO) and recovery strategies outlined in the areas’ business continuity and business resumption plans.
With the command/HICS structure, the Business Continuity Operations Branch will lead BCP activities to:
Facilitate the acquisition of and access to essential recovery resources, including business records (e.g., patient medical records, purchasing contracts).
Support the Infrastructure and Security Branches with needed movement or relocation to alternate business operation sites.
Coordinate with the Logistics Section Communications Unit Leader, IT Unit Leader and the impacted area to restore business functions and review technology requirements.
