These elements must cover the entire workflow that affects the quality of the organization’s products and services. This workflow starts and ends outside of the laboratory. Nevertheless, the laboratory must control the processes, activities, and results from each step of the workflow in order to assure the highest quality.  \nCAP Requirement for a QM Program  \nThe Joint Commission and the CAP’s Laboratory Accreditation Program (LAP) require laboratories to maintain a written plan that describes the laboratory’s overall quality management program. A quality management plan spells out the specific steps that a laboratory will take to ensure that quality is being maintained. In laboratories that are part of a larger institution (eg, a hospital), the laboratory quality management program must be integrated with the institutional program.  \nLaboratory Medical Director Responsibilities  \nCLIA explicitly describes a laboratory director’s responsibilities for ensuring quality in the laboratory: It is the responsibility of the laboratory director \"to ensure that the laboratory develops and uses a quality system approach to laboratory testing that provides accurate and reliable patient test results.\" Integral to the quality systems or quality management approach is the continuous monitoring of each test process (eg, quality control, proficiency testing), taking corrective action, and evaluating the corrective actions to make sure that they were effective in preventing a recurrence.  \nThe laboratory medical director is the person ultimately responsible for the development, implementation, and ongoing monitoring of the quality management program. Of course, the laboratory medical director receives support from other laboratory staff to carry out responsibilities related to the quality management plan. The LMD may delegate some quality management activities but retains the responsibility for ensuring that the quality management program is effective and meets patient care needs and the standards of CLIA and accrediting organizations. Quality can only be effective when the pathology group, the laboratory staff, and the steps of laboratory testing are included in the quality management plan.  \nPatient Care Services  \nPathology is directly involved in patient care by providing information that is essential for the evaluation and management of patient care. High-quality patient care is the ultimate goal of any laboratory. The following is the 2013 CAP requirements that are related to Quality Management of Patient Care Services. (Note: Please check your CAP Accreditation Checklist for the most recent updates. )  \nQM Formats  \nA properly documented and implemented plan will provide reasonable assurance that the laboratory:  \n1. Meets defined standards of quality practice.  \n2. Maintains compliance with applicable laws and regulations related to quality and patient safety.  \n3. Engages in credible quality improvement activities.  \nThere is no specific format for the quality management plan as long as all of the required elements are documented.  \nComponents of a Quality Management (QM) Plan  \nComponents of a QM Plan  \nDue to the complex nature of the laboratory, there are many components that must be addressed when developing a QM plan for your laboratory. These components include:  \n1. Revision history  \nThe first section of the quality plan is the revision history which documents revisions and changes to the quality plan to ensure that the most recent quality plan is being referenced by all laboratory staff.  \n2. Introduction  \nThe Introduction documents the laboratory’s quality goals, the purpose of the QM Program, and also describes the components of the QM Program at a high level.  \n3.
