Supports annual learning by keeping each QI activity in one place and using a step-by-step approach for small or large QI work. Guides teams to apply the Model for Improvement (MFI), including defining the problem, testing small changes, using a whole-team approach, and sharing success and lessons. Includes structured sections for “thinking” (Table 1), “doing” with PDSA cycles, reflection, and celebrating outcomes.","Quality Improvement Record - a template for practices
By documenting Quality Improvement activities, your practices can build on improvements every year. Keeping a record of each QI also helps to keep your team approach and learnings in one place.
NWMPHN has developed this Quality Improvement Record template for practices to use, for any QI activity small or large. This template is a simple step-by-step approach to help you to document your Quality Improvement journey.
If you prefer to use the guided Quality Improvement Toolkits developed by NWMPHN, visit \u0013 HYPERLINK "https://nwmphn.org.au/for-primary-care/quality-improvement/quality-improvement-toolkits/" \u0014nwmphn.org.au/for-primary-care/quality-improvement/quality-improvement-toolkits/\u0015
QUALITY IMPROVEMENT {ADD TITLE}
\u0003TABLE OF CONTENTS
\u0013 TOC "Heading 1,1,Heading 2,2,NWMPHN Appendix heading,1,NWMPHN Headinglevel1NoNumber,1" \u0014
1) Model for Improvement \u0013 PAGEREF _Toc70348709 \\h \u00143\u0015
2) Before you start \u0013 PAGEREF _Toc70348710 \\h \u00143\u0015
3) Part 1: The thinking part \u0013 PAGEREF _Toc70348711 \\h \u00144\u0015
4) Part 2: The doing part \u0013 PAGEREF _Toc70348712 \\h \u00145\u0015
5) QI activity reflection \u0013 PAGEREF _Toc70348713 \\h \u00146\u0015
6) Sharing success and celebration \u0013 PAGEREF _Toc70348714 \\h \u00146\u0015
\u0015\u0004
\u000f
Model for Improvement
This Quality Improvement Record will help you to work your chosen Quality improvement activity.
Each Quality Improvement activity you do should follow the Model for Improvement (MFI) approach outlined below.
The Model for Improvement is a proven approach for developing, testing and implementing changes in general practice. The MFI helps you to break down your change into manageable pieces, which are then tested to ensure that the change results in measurable improvements, and that minimal effort is wasted.
If you are new to quality improvement, it’s a good idea to read the \u0013 HYPERLINK "https://nwmphn.org.au/wp-content/uploads/2020/02/QI-Toolkit_Pip-2018-.pdf" \u0014NWMPHN QI guide and tools\u0015 that provides more information about the stages of the MFI.
Implementation of the MFI has shown that it will work best when you:
Define the problem
Think small and test
Use a whole team approach
Share success and lessons learnt
Before you start
Brainstorm area for improvement
Get your practice team together and think about some areas where you can make improvements.
Use data and evidence to identify where you need to make an improvement, or where there is an opportunity for improvement. For example, you could examine:
• Practice feedback from patients, staff or accreditation
• Data (e.g. routine practice data, CAT4 reports, PIP QI reports)
• Periodic reviews or audits of clinical information
• Local or national health priority areas
Set up the team
Set up a QI team to manage and monitor the activity. Consider:
Who is in the practice?
Who will lead the team?
Who in the practice needs to approve the QI or changes?
How often will you meet?
How will you communicate within the team and with other practice staff?
Part 1: The thinking part
As a QI team complete Table 1.
Table 1: The thinking part
Part 2: The doing part
Considering your ideas for change captured above, use the PDSA template below (Table 2) to document and monitor the effect of your changes. Multiple PDSA cycles are likely to achieve your improvement goal. Remember: PDSA cycles are intended to allow you to test small incremental changes – break your ideas and activities into small chunks. Repeat Table 2 for each PDSA cycle focusing on the goal outlined in Table 1. You can download other PDSA templates \u0013 HYPERLINK "https://nwmphn.org.au/wp-content/uploads/2020/08/NWMPHN-QualityImprovement-PDSA-Template2.pdf" \u0014here\u0015
Table 2. Plan, Do, Study, Act (PDSA)
QI activity reflection
Take a moment to document the QI team reflections on the activity.
Consider: What worked well, what could have been improved, and what will you do differently next time?
CelebratE and Share Your Success
Celebrating your success doesn’t just feel good—it will help you capitalise ","cbCaicj6kBD84Pmr","https://ap.wps.com/l/cbCaicj6kBD84Pmr","docx",1081466,6,"English","en",105,"# Model for Improvement
# Before you start
# Part 1: The thinking part
# Part 2: The doing part
# QI activity reflection
# Sharing success and celebration","[{"question":"What is the purpose of the Quality Improvement Record template?","answer":"It helps practices document quality improvement activities so improvements can build over time, while keeping team learnings in one place."},{"question":"How should a quality improvement activity be structured?","answer":"Each QI activity should follow the Model for Improvement (MFI) approach, breaking the change into manageable parts that can be tested for measurable gains."},{"question":"What framework is used to document and monitor changes?","answer":"Use PDSA cycles—Plan, Do, Study, Act—repeating the table for each cycle to test small incremental changes."}]","Quality Improvement Record - a template for practices | DOCX",1788304241,2
"ok
"quality-improvement-record-a-template-for-practices","",{"@graph":36,"@context":85},[37,53,68],{"@type":38
BreadcrumbList",[40,44,47,50
@type":43
https://docshare.wps.com","Home","ListItem
@type":43
https://docshare.wps.com/template/","Template
@type":43
https://docshare.wps.com/template/forms/",3
@type":43
https://docshare.wps.com/template/quality-improvement-record-a-template-for-practices/173511/",4
@type":54
"DigitalDocument
@type":56},"Person
@type":59},"DocShare","Organization","application/vnd.openxmlformats-officedocument.wordprocessingml.document","2026-09-05","2026-09-01",true,{"@type":65
InteractionCounter",{"@type":67},"ViewAction",{"@type":69
FAQPage",[71,77,81
@type":73
What is the purpose of the Quality Improvement Record template?","Question
@type":76},"It helps practices document quality improvement activities so improvements can build over time, while keeping team learnings in one place.","Answer
@type":73
How should a quality improvement activity be structured?
@type":76},"Each QI activity should follow the Model for Improvement (MFI) approach, breaking the change into manageable parts that can be tested for measurable gains.
@type":73
What framework is used to document and monitor changes?
@type":76},"Use PDSA cycles—Plan, Do, Study, Act—repeating the table for each cycle to test small incremental changes.","https://schema.org
"article
index,follow
[93,98,103,108,113,118,121,126,131
11,"Presentations",90,"presentations
12,"Resumes",80,"resumes
14,"Invoices",70,"invoices
15,"Posters",60,"posters
16,"Social Media",50,"social-media
40,"forms
18,"Letters",30,"letters
21,"Paper Templates",5,"papers-templates
158,"General","general-158"]
