This new guidance focuses on self-advocacy and individual and families’ desire for change and creates a new emphasis on self-determination and choice for individuals receiving services and supports in our system. The Person-Centered Planning process begins with an individual's vision for a preferred life and takes the concept of self-determination from theory to practice. The individual has a primary role in person-centered planning and should be provided the opportunity to participate fully in this process.\nThe purpose of the PCP Guidance Document 2023 is to support Qualified Professionals to employ the knowledge and skills necessary in the planning and development of accurate and effective PCPs.
