Proof of family income must be placed in each child’s school file. During compliance monitoring visits, monitors will look for this documentation.  \nThe following is a list of evidence that may be submitted:  \n● Pay stubs (Two most recent, consecutive)  \n● Proof of WIC benefit, Proof of Supplemental Nutrition Assistance Program (SNAP) benefit  \n● Proof of Temporary Assistance for Needy Families (TANF) enrollment  \n● Proof of Supplemental Security Income (SSI) benefit  \n● Proof the family receives Child Care Assistance Program (CCAP)  \n● The tax return must be the most recent  \n● Wages and tax statements must be the most recent  \n● Verification/letter from employer  \n● Proof that the parent is enrolled in Medicaid (a medical card with the child’s name does not prove income eligibility) .  \nIncome verification must be collected for every child enrolled in the program.
