Local policies and planning efforts should be considered and integrated through social service and emergency management agencies. \nOnce this template has been refined for local use, its implementation should be a collaborative effort between the social worker and caregiver. This approach will ensure both parties have a mutual understanding of the importance of emergency planning, the caregiver can ask clarifying questions, and the correct emergency information and protocols can be clearly communicated with the family. Additional materials have been provided to supplement the template and provide a more comprehensive approach to disaster preparedness in the context out out-of-home placements. \nInstructions: Introduction to the plan should be catered for your agency or organization and logos swapped for your locality. As a best practice, all phone numbers should include the area code. \nThis tool was developed as part of the Resilient Children/Resilient Communities Initiative at the National Center for Disaster Preparedness, Earth Institute, Columbia University’s and supported by a grant from the global healthcare company GSK. \nTABLE OF CONTENTS \nCONTACT INFORMATION ................................................................................................................... 2 \nSocial Network Contacts ..................................................................................................................................................................... 2 \nSafe Location Within Home | Outside Home ...................................................................................................................................... 2 \nFirst Choice, Within the Same Community ......................................................................................................................................... 2 \nSecond Choice, Within the Same Community .................................................................................................................................... 2 \nThird Choice, Outside of Region ......................................................................................................................................................... 2 \nHazard Assessment ............................................................................................................................................................................ 3 \nUtility Shut Offs ................................................................................................................................................................................... 3 \nLocal Non-Emergency Contacts: Work with Agency Staff to Complete.............................................................................................. 3 \nPlanning Checklist .............................................................................................................................................................................. 3 \nAPPENDIX A – EVACUATION ROUTES ............................................................................................. 4 \nAPPENDIX B – CHILDREN WITH SPECIAL HEALTHCARE NEEDS ................................................. 6 \nContact Information ......................................","cbCaidMw6lf6AfJI","https://ap.wps.com/l/cbCaidMw6lf6AfJI","pdf",1346082,"English","# Introduction\n# Table of Contents\n## Contact Information\n### Social Network Contacts\n### Safe Location Within Home | Outside Home\n### First Choice, Within the Same Community\n### Second Choice, Within the Same Community\n### Third Choice, Outside of Region\n### Hazard Assessment\n### Utility Shut Offs\n### Local Non-Emergency Contacts: Work with Agency Staff to Complete\n### Planning Checklist\n## Appendix A – Evacuation Routes\n## Appendix B – Children with Special Healthcare Needs\n## Appendix C – Emergency Contact Cards\n## Appendix D – Foster Child Roster\n## Appendix E – Educational Materials","[{\"question\":\"What is the purpose of the Out-of-Home Placement/Foster Care Emergency \\u0026 Disaster Plan template?\",\"answer\":\"It provides a foundation for an active conversation about disaster preparedness between out-of-home placement agencies and their designated caregivers.\"},{\"question\":\"Why does the template need to be adapted for local use?\",\"answer\":\"It is not comprehensive and should be refined to match local requirements and terminology, and to integrate local policies and planning efforts through relevant agencies.\"},{\"question\":\"How should the plan be implemented after it is refined for local use?\",\"answer\":\"Implementation should be a collaborative effort between the social worker and the caregiver so both parties share understanding, clarify questions, and communicate the correct emergency information and protocols with the family.\"}]","Out-of-Home Placement/Foster Care Emergency & Disaster Plan Template | PDF",1790128549,6]
