This tipsheet will provide instructions for providers to place the orderable, complete the requisition form based in RedCap, and print a consent for testing (requiring patient signature) . This document reviews the workflow for ordering an exome sequencing test. \nSteps To Follow \n1. Open a patient encounter. \n2. Navigate to your order entry area on the Visit Taskbar or Manage Orders Sidebar. \n3. Search for the orderable name. \na. There are three orderable options for exome. \n• Whole Exome Seq: for the proband \n• Whole Exome Seq – Family only Blood: order in the family chart, not the proband \n• Whole Exome Seq – Family only Saliva: order in the family chart, not the proband \n4. Select the correct order and Accept. \n5. Complete all hardstops in the order details window for the exome order type you select. \na. Only blood samples are accepted for proband. Select Blood in the Specimen Type field. \n6. Select Yes for Was Consent Obtained. \na. See below for how to access the consent form on MedEx. \n7. Research consent and Secondary findings consent are optional. Select Yes or No as appropriate. \n8. Complete the Red Cap Survey Whole Exome Order Information under the Reference Links section. \n\n| | For the order to be placed, you must do the following:\u003Cbr>• Obtain verbal and written consent for testing (see below on how to access consent via MedEx ) .\u003Cbr>• Research consent and secondary findings consent are optional.\u003Cbr>• Complete the RedCap Survey. |\n| --- | --- |\n\n9. Click the blue link under Reference Links for the Red Cap Survey. This is like a test requisition. \na. The survey can be started and finished later in the day if needed by clicking the Returning button at the top. \nb. This will allow you to send yourself the partially completed survey via email. \nc. You will need to complete section one (at least) before opting to return to the order. \n10. If you are testing family members (duo or trio based test), you will need to select the number of family members to be tested. \na. This will automatically populate orderable information for family members so samples can be linked. \n11. If family members do not have MRNs, these will need to be created before signing the orderable for the proband. \n12. If samples for family members will be drawn a different day or saliva will be used, mark No for Will sample be collected during this visit. \n13.After completing all hardstops and addressing the survey, click Accept. \n14. Select Sign Orders. \n15.Associate the order to a diagnosis. Select the ICD-10 code that should be used for billing. \n16. Print the lab consent form from MedEx. \na. Select MedEx from your Hyperspace toolbar across the top of the screen, from within your Epic menu drop down, or using the search box in the upper right corner under Log Out. \n17.Within MedEx, search for the patient by MRN, Name, or Encounter number in the top box. \n18. Select the correct patient and use the right pointing arrow to move the patient into the Selected Patients section. \na. You can also double click on the patient row to add to the right side. \n19. Search in the bottom Forms section for Consent Exome Sequencing. \n20.Move the correct form to the Selected Forms section. \n21.Click Preview or Print to print out the consent form with the patient identifiers. \n22.Once the form is signed, scan the form into eStar using your normal scanning process. \n\n| | Uploading the signed consent form into eStar is a required component of this workflow. |\n| --- | --- |","cbCaijNrjzDwzt3R","https://ap.wps.com/l/cbCaijNrjzDwzt3R","pdf",638632,"English","# Steps To Follow\n## Order entry and selecting the correct exome orderable\n## Consent, RedCap survey, and accepting the order\n## Sign orders, ICD-10 association, and printing consent","[{\"question\":\"Which Whole Exome Sequencing orderable should be selected for the proband versus family members?\",\"answer\":\"Use Whole Exome Seq for the proband.
