This individual must be a Licensed Vocational Nurse (LVN) . The Nurse Case Manager is responsible for initial and ongoing implementation of a service plan. Also, coordinates and monitors cost effective, quality services provided in accordance with the client’s needs as set forth in the service plan.  \nWith a team of caring and committed individuals and hundreds of dedicated volunteers, RHC is working to eliminate the stigma and overcome health disparities impacting the Lesbian, Gay, Bisexual, Transgender, Queer + (LGBTQ+) community by providing healthy and compassionate care options.  \nRHC is the largest Orange County provider for such healthcare services and is currently expanding its capacity with plans to create a dedicated Federally Qualified Health Center (FQHC) for the LGBTQ+ community.  \nThe Nurse Case Manager will:  \n• Perform comprehensive assessments of physical, psychological, social, spiritual, environmental, financial and functional independence.  \n• Assess and evaluate prospective clients for the various programs available to the client. Admit prospective clients based on application of the various criteria for the specific program.  \n• Obtain and maintain necessary documentation regarding client’s medical and demographic data, including but not limited to, necessary consents.  \n• Obtain written verification of symptomatic HIV or AIDS diagnosis from client’s attending physician.  \n• Develop, implement, monitor and modify a service care plan through an interdisciplinary team process.
