Adjournment  \nServices Agreement  \nThis Services Agreement (“Agreement”), dated as of January 1, 2021 (the “Effective Date”), is entered into by and between ENTER NAME OF AGENCY an Illinois entity, with offices located in Bloomington, Illinois (“Agency”) and the Board of Health, the governing body of the McLean County Health Department, with offices located at 200 W. Front Street in Bloomington, Illinois (“Board”) .  \nWHEREAS, there is a need for IDENTIFY BEHAVIORAL HEALTH NEEDED, located in McLean County IL; and  \nWHEREAS, AGENCY has the capacity to provide such a service; and  \nWHEREAS, the BOARD having been appointed by the County Board Chair as the Administrators to oversee certain funds allocated by the County of McLean for behavioral health programs and activities; and  \nWHEREAS, the BOARD desires to retain the AGENCY to provide the said services under the terms and conditions hereinafter set forth, and Service Provider is willing to perform such services;  \nNOW, THEREFORE, in consideration of the mutual covenants and agreements hereinafter set forth and for other good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged, AGENCY and the BOARD (hereinafter, collectively, the “Parties”, or each, individually, a “Party”) agree as follows:  \n1.
