Management of Change (MOC) is a process that systematically identifies, evaluates, and manages changes to facilities, equipment, operations, procedures, and the organization in order to prevent incidents by ensuring that unacceptable risks are not introduced into the business. \nThe Health & Safety Management System (HSMS) MOC Standard sets the minimum requirements for MOC in Pacific Gas and Electric Company (PG&E) . \nThis Standard applies to all PG&E locations, premises, facilities , and work activities. \nLine of Business (LOB) MOC procedure(s) must meet the requirements of this standard. \nTARGET AUDIENCE \nAll PG&E senior and executive leadership, line of business (LOB) leaders and PG&E personnel, collectively described as Pacific Gas and Electric, Co. (“PG&E”) . \nTABLE OF CONTENTS \nSUBSECTION \nTITLE \nPAGE \n1 Purpose............................................................................................................. 2 \n2 Types of MOC ................................................................................................... 3 \n3 Replacement in Kind ......................................................................................... 4 \n4 MOC Process .................................................................................................... 4 \n5 Competencies and Training............................................................................... 5 \n6 Roles & Responsibilities .................................................................................... 5 \nAppendix A, Implementing Standards and Procedures ...................................................... 10 \nRequirements \n1 Purpose \nThe purpose of the MOC process is to manage hazards and risks that arise from changes to facilities, equipment, operations, procedures, and the organization. The MOC process supports personnel to manage changes such that: \n1. Proposed changes are screened and prioritized. \n2. The hazards and risks of the change are understood and mitigated. \n3. An appropriate level of risk assessment is completed. \n4. The change is authorized by appropriate authorities and/or appointed approving authority. \n5. The change is designed to the appropriate standards. \n6. Changes are communicated, and training, where applicable, is provided to those impacted by the change. \nThe Standard applies to: \n1. Equipment, including safety and emergency equipment \n2. Hardware and software, measurement, control, and data acquisition systems that control and monitor plant operations including process alarms. \n3. Building, facility, or infrastructure (permanent and temporary), including location, design, and layout \n4. Operating and maintenance procedures, including changes to IT or other systems that control plant operations. \n5. Exceptions to approved process, procedures, standards, or specifications. \n6. Organizational changes The following are out of scope: \n1. Replacement in kind, (RIK), also known as like-for-like \n2. Changes that are addressed by other existing processes, procedures, codes, or regulations, provided that the change is risk-assessed \n3. Changes to process variables within a well-defined operating envelope (e.g. process controller set points) \n4. Financial and Information Technology (IT) systems that do not control and/or monitor plant/facility operations \n5. Other administrative and commercial procedures \nThe above criteria should be used to screen proposed changes prior to applying the MOC process. \n2 Types of MOC \nThis Standard applies to permanent, temporary, emergency, and organizational changes. LOB must document and define the following change types: \n2.1 Permanent Change \n1. A permanent change is a modification that alters the original design to assets, facilities, operations, technology, equipment, procedures, organization, and safety critical workforce and does not mee","cbCaijmismH2YbZY","https://ap.wps.com/l/cbCaijmismH2YbZY","pdf",228198,10,"English","# 1 Purpose\n## Manage hazards and risks from changes\n# 2 Types of MOC\n## Permanent, Temporary, Emergency, Organizational changes\n# 3 Replacement in Kind\n## Out of scope criteria\n# 4 MOC Process\n## Initiation and phase-based procedure requirements\n# 5 Competencies and Training\n# 6 Roles & Responsibilities\n# Appendix A, Implementing Standards and Procedures","[{\"question\":\"What is the purpose of the MOC process?\",\"answer\":\"The MOC process manages hazards and risks arising from changes to facilities, equipment, operations, procedures, and the organization, by screening and prioritizing proposals, assessing and mitigating risks, and ensuring authorization, standards compliance, and communication/training.\"},{\"question\":\"What types of changes does the standard cover?\",\"answer\":\"The standard applies to permanent, temporary, emergency, and organizational changes, with definitions for each type and requirements for how LOBs document and define them.\"},{\"question\":\"When is replacement in kind (RIK) excluded from MOC?\",\"answer\":\"Replacement in kind activities are out of scope for the MOC process when the replacement is technically like-for-like, fully conforms to approved specifications, and has no impact on the system.
