They will have direct line responsibility for the Clinical Leads, the Chronic Disease Nurse Specialists, the Pharmacists and the Care Coordinators. The team as a whole also includes Paramedics, Practice Nurses, Nurse Associates, Trainee Nurse Associates and Healthcare Support Workers who all have a named Lead.  \nThe post holder will hold an advanced clinical qualification and current registration with a professional body. They will also hold a non-medical prescribing qualification to help understand and support the needs of our clinicians who also hold this qualification, or wish to achieve it.
