This includes providing emotional support, assistance with navigating health and social services, and connecting caregivers with community resources tailored to their needs, such as mental health, financial support, housing, social opportunities, exercise, and nutrition. The Caregiver Navigator will build meaningful ongoing relationships with caregivers, co-create wellness plans, and use motivational interviewing and action planning to empower caregivers in reaching their wellness goals.
