An􀆟bio􀆟c Name*: Select an an􀆟bio􀆟c from the drop-down list. An􀆟bio􀆟cs are listed in the following format: 'Generic name (brand name)' . If the an􀆟bio􀆟c isnot in the drop-down list, choose “Other.”  \n1. Other An􀆟bio􀆟c: If you selected “Other” in the previous column, specify the name of the other an􀆟bio􀆟c here.  \nii. Indica􀆟on* : Select the indica􀆟on for the an􀆟bio􀆟c prescrip􀆟on from the dropdown list. If the indica􀆟on nis not in the drop-down list, choose “Other.”  \n1. Race of Prescribed Resident*: Enter the race of the prescribed resident.  \n1. Race 2-7: A resident may have more than one race.
