The form must reflect all members of the family: Name, DOB, School , ID number and Grade . The form MUST reflect all school age children in the home and any other members of the family that the \"provider\" is taking care of, so elderly members of the family that are 100% cared for by the \"provider\" need to be on the list as well. Please leave the school and grade fields blank for any adults.  \nThen you check your total income on your tax return and look at the box at the bottom of the Income Survey form to see if your family does, indeed qualify for assistance. This decision is based on the total number of people in the home and the federal guidelines to qualify with your families tax return showing the total income.  \nIf you meet the criteria then you check the box and please return the completed form with a copy of the 1st page of the tax return that shows the total income. You can email me with photos of the form and tax return page, please redact (black out) any confidential information such as social security numbers if emailing or bring them into school, I will return them to you.  \nWe will look at the single page of the tax return that shows total income ( IRS Tax form-1040 , line 11) and give it back to you. We need proof that what you have shared on the Income Survey is accurate. If you live in a household that does not file taxes because there is no income then we need to see documentation that shows you have no income like Supplemental Security Income (SSI) paperwork.
