Both parties commit to review progress and sign off adjustments, with the support process subject to consent and coordination when needed.","Sue Ryder\nInclusion Passport\n\u000f\nWhat is an Inclusion Passport?\nThe health and wellbeing of our staff is our number one priority. We are committed to supporting colleagues to look after themselves and each other. The Inclusion Passport will help you explore your own needs and help guide honest and supportive conversations with your manager; making them aware of anything that is going on in your life that might require workplace adjustments.\nThe passport is there to help you thrive at work. For example, it may enable you to successfully continue working while managing a health condition or caring for a loved one. The passport also reduces the need for conversations to be repeated and arrangements to be re-agreed if you change manager.\nWho is the passport for?\nFor anyone who wants to have a conversation with their manager about what support they need to be at their best at work. The Inclusion Passport is also for managers, who have a responsibility to understand, engage with and promote the inclusion passport. (Please look at the Manager Guidance.)\nThe passport might not work for everyone. You might communicate your needs in a different way. Whether you use the passport or not, as an organisation we’ll support you and work with you to find an approach that meets your needs.\n\u000f\nBenefits of the passport:\nProvides you and your manager with a template for having an open conversation and a transparent record of arrangements that have been agreed\nReduces the need to renegotiate arrangements with a new manager\nProvides a clear timeline for you and your manager to review how the arrangements are working\nProvides you with an opportunity to suggest adjustments or arrangements, and to decide with your manager what will work best given the demands of your role\nProvides you with the confidence that your manager will know how to respond when there are changes in circumstances. For example, if your health gets better or worsens, or the person you’re caring for becomes more independent.\nHow should I use the passport?\nThe template below can form the basis for an open, confidential discussion between you and your manager. The passport can be completed in a way that works for you. You can either work on the passport independently and then use it to guide the conversation with your manager, or you can work on it with your manager - communicating and completing it at the same time.\nThe agreed adjustments should be signed off by both parties. Your manager may have to consult their manager, our People team, and/or Occupational Health before certain adjustments can be implemented. However, this won’t be done without your consent and you’ll be kept updated throughout this process.\nIf you change roles at Sue Ryder, you can show the passport to your new manager. Your new manager should continue the adjustments that have previously been put in place unless there are clear reasons that prevent this from being possible. In this case, the template can provide useful insight into what has worked well previously and, again, form the basis of a supportive discussion. With your consent, your previous manager should have a conversation with your new manager focusing on the adjustments in place and what they have done to ensure that they work well. This will ease the transition between managers.\nThe passport is for you to keep and can be stored on the People Hub if you wish. If you store the Passport in People Hub and you change managers, your new manager will be able to see your passport. To store your passport, go to your profile and select ‘documents’ on the left hand side. Under ‘Document Type’ select ‘Inclusion Passport’\nThere are many factors, including your identity, that influence your needs\nSome examples include: a parent or carer needing additional support; a colleague needing flexible hours during a religious festival; a colleague experiencing the menopause who needs additional equipment; a trans colleague needing time off during parts of their transition.
