This therapy may also be referred to as viscosupplementation.  \nOsteoarthritis is a degenerative condition of the joints and is the most common form of arthritis. Osteoarthritis commonly affects the hands and the weight-bearing joints including the knees, hips, feet and spine. Afflicted joints experience loss of synovial fluid, a protective substance which aids in absorbing shock and lubrication in the joints. Low synovial fluid levels and other mechanisms cause a progressive breakdown of the cartilage lining the ends of bones that are necessary for proper cushioning and smooth function of joints. Because of this breakdown of cartilage, bones rub against each other causing pain, loss of movement and further destruction of the joint. The severity of osteoarthritis can range from very mild to very severe. There is no cure for osteoarthritis and current treatment focuses on relieving symptoms and improving function. The hyaluronan injections are considered to be a device by the United States Food and Drug Administration and several have been approved via the premarket approval process. Proposed use of hyaluronan for osteoarthritis of the knee is derived from demonstration of benefit of varying degrees in a number of trials and meta-analyses of randomized trials. However, many analyses have not shown a clinical benefit beyond the effect seen with placebo.  \nThe 2012 Annals meta-analysis by Rutjes and colleagues was a pivotal piece of evidence underpinning later guidance that moved away from hyaluronic acid (HA) for knee OA. In 89 trials (12,667 patients), the authors showed that any pain benefit with viscosupplementation was reduced to a level that was not clinically meaningful when analyses were limited to the larger, better-blinded trials (effect size ~0 .11 SD) . To put this in perspective, the minimal clinically important difference (MCID) for pain in knee osteoarthritis is generally considered to be about 0.37–0.40 SD, or roughly a 10–20% reduction on a pain scale—the threshold at which patients actually perceive improvement. The effect of HA fell well below this bar, suggesting that even though statistical differences were sometimes seen, they were not large enough to translate into a noticeable benefit for patients. The study also identified signals of publication bias and a higher risk of adverse events versus control and concluded that use “should be discouraged.” These findings align directly with the ACR/Arthritis Foundation 2019 guideline, which issued a conditional recommendation against intra-articular HA for knee OA because benefits were limited/inconsistent and did not justify routine use, reserving it only for exceptional, preferencesensitive cases after shared decision-making. They also closely mirror the conclusions of the NICE 2022 NG226 guideline, which states“Do not offer intra-articular hyaluronan injections to manage osteoarthritis”—a stance NICE reached from clinical evidence syntheses showing minimal benefit (consistent with Rutjes et al. ) and from cost-effectiveness considerations in the UK setting.  \nIn 2015 meta-analysis, Bannuru and colleagues reported on the efficacy of treatments for osteoarthritis of the knee evaluating pain, function and stiffness. A total of 137 studies made up of 33,243 participants were included in the analysis.
