If you signed up for electronic delivery, your tax forms will be available on the Member Website and you will not receive a paper tax document in the mail.  \nIRS Form 1099-SA (Distributions)  \nIf you have distributions from your HSA, you should have received this form in January. It can be located on the Member Website on the “Statements & Notifications” tab. If you did not have distribution activity in 2019, you will not receive an IRS Form 1099-SA.  \nIRS Form 5498-SA (Contributions)  \nThis form provides you with all the contributions made to your HSA in 2019. The 5498-SA will be available in May 2020, allowing any contributions made in 2020 for 2019 to be included. If you did not have contribution activity for 2019 or a balance (Fair Market Value) as of December 31, 2019, you will not receive an IRS Form 5498-SA.  \nHow to view your contributions by tax year  \nTo view your contributions prior to receiving the 5498-SA form, you can access this information on the Member Website by following the below steps.  \n1. Log into the Member Website and click on the “Accounts” tab.  \n2. Click on “HSA Contributions By Tax Year” in the left column navigation.  \n3. A window will appear showing your HSA contributions by tax year.  \nTax Year 2019 Example:  \nIRS Maximum Amount*: The maximum annual HSA contribution for 2019.  \nContributions: The total contributions to your HSA made in 2019 (Box 2 of 5498-SA tax form) . This includes:  \n• Contributions made in 2019 for tax year 2019  \n• Contributions made between January 1 and April 15, 2020, designated for 2019  \n• Contributions made for 2019 that you have since withdrawn as an Excess Contribution  \n• Any contribution you directly transferred from an IRA in 2019  \nContributions from Future Years: The total contributions made in 2019 for 2018 tax year.
