Outreach Guide  \nPurpose: To help plan, organize, and follow up on outreach to health care provider (HCP) practices, including APPs (i.e., nurse practitioners, physician assistants), to facilitate consistent referrals.  \nAudience: Staff responsible for recruiting participants into their National DPP LCP  \nUse: Read this guide first and use it as the foundational document for your planning and outreach. It refers to most of the other supporting materials.  \n2. LCP Needs Assessment Tool and Outreach Plan Template  \nPurpose: To help LCP delivery organizations 1) identify how they want to engage HCPs and APPs, specifically, in referrals and what they have in place or still need to be successful, and 2) create a plan for their outreach.  \nAudience: LCP delivery organizations, including LCP recruitment staff  \nUse: You can fill out this tool electronically or print it. Consider including all the people in your organization who have a role in your LCP in a discussion about the questions in the tool. If you or your organization cannot answer the questions in this tool, consider using the HCP Engagement Planning Checklist (see below) .  \n3. Health Care Provider (HCP) Engagement: Planning Checklist   \nPurpose: To help LCP delivery organizations learn what they should consider or have in place to successfully engage HCPs and APPs, specifically, in referrals.  \nAudience: LCP delivery organizations, including LCP recruitment staff  \nUse: For organizations that find it difficult to answer the questions in the LCP Needs Assessment Tool.  \n4. How to Present Your Program Tip Sheet   \nPurpose: To help LCP delivery organizations have successful conversations about their programs with APPs and other health care practice staff.  \nAudience: Staff responsible for recruiting participants into their National DPP LCP  \nUse: You can review this tip sheet prior to speaking about your program to others. It provides helpful tips and a sample elevator speech that you can tailor for your program.  \n5. National Program Factsheet   \nPurpose: To provide APPs with information they want to know about the National DPP and how they can contact their local LCP.  \nAudience: APPs and other HCP practice staff  \nUse: You can add your program’s contact information at the end of this factsheet and share it with APPs to educate them about the National DPP. It includes the information APPs told us they want to know about the National DPP before deciding to refer patients.  \n6. National and Local Program PowerPoint Slides  \nPurpose: To provide an overview of the National DPP and an APP’s local LCP.  \nAudience: APPs and other HCP practice staff  \nUse: You can use these slides for a presentation or pull them up on a laptop for a 1:1 meeting. You can also print them and put them in a binder for office visits, meetings, or to use as a leave-behind for a provider or practice. These slides include the information APPs told us they want to know about the National DPP and their local LCPs before deciding to refer patients.  \n7. Patient Handout (for various patient audiences)  \nPurpose: To provide APPs with a resource they can give their patients about the National DPP and how to contact their local LCPs.  \nAudience: APPs or other HCPs, for their patients at risk for diabetes  \nUse: You can add your program contact information to the bottom of the handout and edit the quote. Multiple versions of the patient handout were created to reflect different patient audiences. Choose the version(s) of the handout that best reflect(s) the patient populations that your APP or other HCP contacts see regularly.
