Documentation found in the clinical records must also be specific to changes in the patient/family’s status and show evidence that all the members of the interdisciplinary group participated in each patient specific written plan of care. \nDocumentation must clearly communicate observations and the reporting of patient status and the care or services furnished as well as the patient’s response. \nDOCUMENTATION TO SUPPORT HOSPICE ADMISSION: \nPatient record must support documentation in technical elements consistent with a terminal prognosis of 6 months or less. Documentation to support patient’s eligibility for the hospice benefit must include clear representation in the clinical record of: \n􀁸 Worsening clinical status to initiate the hospice referral \n􀁸 Decline in clinical status \n􀁸 Diagnostic findings, Laboratory testing to support terminal illness \n􀁸 Functional status decline \n􀁸 Co‐morbidities \n􀁸 Date of diagnosis and course of illness \n􀁸 Patient has desire for palliative, non‐curative treatment \n􀁸 Recurrent or intractable infections such as pneumonia, sepsis, or upper urinary tract infection \n􀁸 Progressive quantifiable decline as documented by weight loss not due to reversible causes (depression, diuretics), decreasing anthropomorphic measurements (mid‐arm circumference, abdominal girth), not due to reversible causes (depression, diuretics), decreasing serum albumin or cholesterol, dysphagia leading to recurrent aspiration and/or inadequate oral intake documented by decreased food portion consumption \nDOCUMENTATION TO SUPPORT CONTINUED ELIGIBILITY (DOCUMENTING DECLINE): \nWe are responsible for assessing for all body systems and treating the whole patient, not just the symptoms related to the terminal illness. We may not be required to pay for medications, and/or treatments considered not related to the terminal illness, but we are still required to care for and assess the whole patient and acknowledge how unrelated issues are being addressed. \nNegative charting demonstrates that we are noticing and treating those symptoms that are present at the terminal phase of one’s life. We know they are there: seek them out, collaborate with your fellow team members about them; document their presence, and their persistence. Finally, make sure your documentation clearly shows your patient was: assessed by a professional, cared for by a capable and competent clinician, the patient’s response to each of your interventions is clearly visible in your documentation and the clinical record shows collaboration amongst members of the interdisciplinary group. \n1 | P a g e Hospice Eligibility Documentation Tips and Strategies 2 \nInstead of “Non‐ambulatory”: \n􀁸 Slumped over in chair \n􀁸 Head resting on shoulder, chest \n􀁸 Extremities are flaccid \n􀁸 Only out of bed for meal(s) \n􀁸 Leans to one side \n􀁸 Requires lap buddy \nInstead of “Wheelchair bound”: \n􀁸 Total lift to wheelchair, pivot, Hoyer lift (explain how the patient got to the wheelchair) \n􀁸 Describe activity of patient while in the wheelchair (motionless, slumped, propels with feet) \nInstead of “Ambulatory”: \n􀁸 Ambulatory with 2 person assist \n􀁸 Utilizes cane or walker to ambulate \n􀁸 Ambulates feet with a recovery time of minutes \n􀁸 Noncompliance with use of cane/walker \n􀁸 History of falls \n􀁸 Shuffles feet when walks \n􀁸 Increased fatigue with activity as evidenced by \n􀁸 Decreased endurance / SOB with ambulation as evidenced by \n􀁸 Unsteady gait \n􀁸 Steadies self when ambulating by holding onto wall/furniture \nInstead of “Bedbound”: \n􀁸 Lying in bed motionless \n􀁸 Time spent in bed is (increasing, decreasing) \n􀁸 / 24 hours spent in bed \n􀁸 No spontaneous movement \n􀁸 Contracted \n􀁸 Does","cbCaitRROcBI8BZD","https://ap.wps.com/l/cbCaitRROcBI8BZD","pdf",166723,6,"English","# Hospice Documentation Requirements\n## Documentation to Support Hospice Admission\n## Documentation to Support Continued Eligibility (Documenting Decline)\n## Wording Tips for Common Patient Descriptors","[{\"question\":\"What must hospice clinical records document to support the plan of care and visit frequencies?\",\"answer\":\"The record must support the plan of care and visit frequencies using findings from comprehensive assessment updates, including evidence that the interdisciplinary team participated in the patient-specific written plan of care.\"},{\"question\":\"What documentation is required to support hospice admission eligibility?\",\"answer\":\"The patient record must support terminal prognosis of 6 months or less and include evidence such as worsening/declining clinical status, diagnostic and laboratory findings, functional decline, comorbidities, diagnosis date and illness course, preference for palliative non-curative treatment, progressive measurable decline, and recurrent or intractable infections.\"},{\"question\":\"How should continued eligibility be documented during a decline phase?\",\"answer\":\"Continued eligibility documentation must assess and treat the whole patient across all body systems, document symptoms present in the terminal phase, show collaboration among team members, and clearly reflect professional assessment and the patient’s response to interventions.\"}]","HOSPICE ELIGIBILITY DOCUMENTATION TIPS AND STRATEGIES | PDF"]
