( For DMEPOS, CMS requires use of the terminology “Corrective Action Plan (CAP) .”) To document and submit your POC/CAP, you must use the ACHCPOC/CAP template and access information on your Summary of Findings (SOF) . Your POC/CAP must address everything identified as deficient on the SOF.
General Instructions
Organizations seeking accreditation must submit their POC/CAP and any required supportive evidence by the deadline provided by ACHC.
Key Points
􀀟 The POC/CAP template must be completed and submitted in Word format.
» Do not convert the form to a PDF or any other format. Additionally, do not print it, write on it, scan it, and then email/fax it to ACHC.
􀀟 The gray columns on the far right of the POC/CAP form are for ACHC internal use only. Please do not insert or delete information in these columns.
􀀟 ACHC includes standard numbers from your SOF on the POC/CAP template. Do not rearrange or delete these standard numbers. They are in the same order as the standards on your SOF and must all be addressed.
􀀟 On the POC/CAP template, you do not need to address anything listed as a “best practice” on your SOF, regardless of whether the standard also has a deficiency.
􀀟 If the SOF indicates that a deficiency was corrected onsite during your survey, you must still describe monitoring of corrective
actions under the “Process to Prevent Recurrence” column on the POC/CAP template. You do not need to insert the plan of correction/corrective action plan, date of compliance, or title.
Filling Out the Form
The POC/CAP template is designed to simplify the process and help you organize your correction plan. The sample below details information that must be provided under each category (column header) to address individual standard deficiencies.

| ACHC Standard | Plan of Correction/Corrective Action Plan
(Specific action taken to bring standard into compliance) | Date of Compliance
(Date correction to be completed) | Title
(Individual responsible for correction) | Process to Prevent Recurrence
(Describe monitoring of corrective actions to ensure they effectively prevent recurrence) | POC/CAP Compliant (ACHC internal use only) | Comments (ACHC internal use only) |
| --- | --- | --- | --- | --- | --- | --- |
| DO NOT change the order of or | 􀀟 Insert a detailed description of your planto become compliant. | Insert the date you will comply with the | Insert the title (not the name) of the | 􀀟 Insert your plan to prevent falling out of compliance with | DO NOT type in or delete anything | DO NOT type in or delete |
| delete any | 􀀟 Address everything mentioned deficient | standard (15-45 days | person | this standard again. | from this column. | anything from |
| standards. On your SOF, submit evidence only for standards listed under the category ** Evidence Required on POC/CAP ** . The POC/CAP template will also indicate that evidence is required under the standard number in the “ACHC Standard” column. No other evidence is required unless specifically requested in the SOF or by ACHC.
􀀟 Evidence does not need to be in Word format.
