This technique is designed to be used with arthritic knees that do not have significant bone loss, major contractures or soft tissue laxity. \nThis technique focuses on knee alignment and in setting tibial and femoral resections based on bone references. Soft tissues are then managed “around”the resections. The technique can be used in both tibia first and femur first approaches. \nThe surgical technique involves extramedullary (EM) alignment for the tibial cut and intramedullary (IM) alignment for the femoral cut. The distal femoral resection is made with respect to the intramedullary canal. The surgeon has the option of making the distal cut with different degrees of valgus relative to the anatomical axis of the femur, in accordance with the patient’s anatomy. In general, patients will tolerate a knee that may be in slight varus, but are generally unhappy if the knee appears to be in valgus. In practice, the authors typically make the cut in 3 to 5 degrees of valgus. \nThe A/P 4in1 cutting block positioning can be based on a choice of anatomical references (posterior condyles, epicondyles or Whitesides line) . \nC A U T I O N \nSome instruments are fixed to the bone using dedicated pins. Before using the pins, ensure that they are intact and fully functional. BENT OR DEFECTIVE PINS CAN NOT BE USED AND MUST BE REPLACED BY NEW ONES. \nWhen extracting pins it is important not to bend them. This results in axial alignment between the pin and the dedicated extractor. \nIt is strongly recommended not to impact or hammer on any instruments unless otherwise specified in the surgical technique. \nFor detailed instructions contact your local Medacta sales representative. \nS Y M B O L S \nThroughout the surgical technique you will find the following symbols: \nOPTION \nThe Option box descriptions describe alternative ways to perform the same procedure. \nSS \nThe MSS box descriptions refer to instruments that have been specificially designed for Muscle Sparing approaches. \nP L E A S E \nN O T E \nIn the surgical technique described hereafter, the resections are performed in the following order: \nA. Distal femoral resection \nB. Tibial resection \nC. A/P femoral resections and chamfers \nNevertheless, the surgeon can change the order of the resections, choosing the following sequence: \nB. Distal femoral resection \nC. A/P femoral resections and chamfers \nA. Tibial resection \nIt is compulsory to perform the distal femoral resection before the A/Presection and chamfers \n3 \n| GMK SPHERE Surgical Technique Hip | Knee | Spine | Navigation |\n| --- | --- | --- | --- |\n\nT A B L E \nO F C O N T E N T S \n1 INDICATIONS 6 \n2 CONTRAINDICATIONS 6 \n3 PREOPERATIVE PLANNING 6 \n3.1 Radiological planning 6 \n3.2 Clinical planning 6 4 SURGICAL APPROACH 6 5 DISTAL FEMORAL RESECTION 7 \n5.1 Placing the intramedullary rod 7 \n5.2 Positioning the distal cutting guide 8 \n5.3 Setting the distal femoral resection level 8 \n6 TIBIAL RESECTION 11 \n6.1 Assembling the extramedullary guide 11 \n6.2 Setting the tibial varus/valgus with the extramedullary guide 12 \n6.3 Setting the tibial slope with the extramedullary guide 12 \n6.4 Setting the tibial resection level with the extramedullary guide 13 \n6.5 Fixation of the tibial cutting block 14 \n6.6 Removing the extramedullary guide 15 \n6.7 Tibial resection 15 \n7 EXTENSION GAP CHECK 16 \n8 ANTERIOR CUT, POSTERIOR CUT AND CHAMFERS 18 \n8.1 Posterior referencing: femoral sizing and external rotation adjustment 19 \n8.2 Posterior referencing: 4in1 cutting block positioning 20 \n8.3 Femoral upsizing/downsizing ","cbCaivYD1BvoS1Ii","https://ap.wps.com/l/cbCaivYD1BvoS1Ii","pdf",1460900,36,"English","# INTRODUCTION\n# CAUTION\n# SYMBOLS\n# TABLE OF CONTENTS\n## INDICATIONS\n## CONTRAINDICATIONS\n## PREOPERATIVE PLANNING\n## SURGICAL APPROACH\n## DISTAL FEMORAL RESECTION\n## TIBIAL RESECTION\n## EXTENSION GAP CHECK\n## ANTERIOR CUT, POSTERIOR CUT AND CHAMFERS\n## TRIAL REDUCTION\n## FINISHING THE FEMUR AND TIBIA\n## PATELLA\n## SELECTION OF THE PROSTHETIC COMPONENTS - SIZE MATCHING\n## FINAL IMPLANT\n## IMPLANTS NOMENCLATURE\n## COMPATIBILITY CHART","[{\"question\":\"What patient and knee conditions is the GMK SPHERE surgical technique designed for?\",\"answer\":\"The technique is intended for arthritic knees without significant bone loss, major contractures, or soft tissue laxity.
