There are no exceptions to this document. If this is not included in the packet of information, the application will be considered incomplete and will not be furthered in the process.\nDear Applicant,\nThis form is the first step of the application process for the fall 2023 dental hygiene program. It is important that you read and complete this form to be able to move to the next step in the application process. If this document is not listed on file, then the application associated will be considered incomplete and will not be processed for consideration into the 2023 Cohort of students.\nThis document is necessary for each applicant to sign, due to the fact that we are in the process of receiving the status of “Initial Accreditation” from the Commission on Dental Accreditation (“Commission” or CODA). Palo Alto College (PAC) cannot officially enroll students into the program until the program receives this designation. However, we can develop a plan to have a class of 30 students aligned once this designation has been received.\nOn April 18 and 19, 2023, the Dental Hygiene Program had their site visit from the members of the Commission. This site visit resulted in the highest level of acknowledgement that can be given to a dental hygiene program. However, the Commission has to officially send notification once the members vote to approve the program. This will not come until late August. Therefore, a contingency plan has been developed to be proactive for the start of the program.\nAt this time, the tentative plan is to begin with an orientation to the program for those applicants who will be selected. This will involve accepting 30 students with 15 alternate positions. All applicants MUST attend the orientation, or they will forfeit their position. No exceptions to this rule. Upon receipt of the official notification from CODA, enrollment will take place and the curriculum will begin. Payment is due at this time for tuition and other supplies.\nPlease read the following information:\nContingency is defined as “a circumstance which is possible but cannot be predicted with certainty.” Acceptance into the program is dependent upon Palo Alto College Dental Hygiene Program receiving the status of \"Initial Accreditation\" from the Commission on Dental Accreditation.
