The complexity of a campus like Utah State University depends upon the participation of the entire community to successfully implement Utah State University’s Emergency Management Plan. Department Emergency Action Plans (EAPs) are an integral part of the overall campus preparedness, response, and continuity of operations program.\nDepartments/Colleges throughout the University are strongly encouraged to develop their own Emergency Action Plans for practical reasons. Each unit best understands the nature of its work, potential work place hazards, facility layout, and special needs of the department (i.e., people with disabilities, research, patients, and animals, etc.).\nThe EAP should address preparedness measures and emergency response. The EAP is a way for your department to plan for potential emergencies such as small accidents; citywide disasters; power outages; hazardous chemical spills; fires; a civil disturbance; or an earthquake. Advanced planning and completion and routine review of a Department EAP will help reduce risk and loss of life. It is important that all faculty, staff, and students read and understand their work site emergency action plan.\nThis form is:\nA flexible tool designed for departmental planning. “Departmental” is loosely defined as any unit of the campus: a college, an academic department, an organized research unit, an administrative or financial unit, etc.\nEasy-to-use and requires no advance training. However, we strongly recommend that you begin your project by inviting the campus Emergency Manager to conduct a brief orientation for your planning group.\nTo submit or update a plan, use this form to:\nIdentify your unit, unit work site evacuation protocols and emergency meeting location(s); personnel assignments, special needs, and contact information; communication and notification protocol; supplies and equipment; and critical academic, service, and business functions.\nCompile Emergency Pocket Cards and Emergency Response and Evacuation Information Sheets, post or distribute to members of your unit, and notify unit employees about contents of the EAP.\nSubmit the EAP to the Emergency Management Department, update and test the EAP annually, and complete a Business Continuity Plan.\nTIPS—BEFORE YOU BEGIN\nFor all but the tiniest units, forming a planning group and having meetings is recommended as well as designating Department Team Leader(s) for the group. (Begin you project by inviting the campus Emergency Manager to conduct a brief orientation for your planning group).\nYour Department Team Leader (or other designee) will typically enter information into this template outside of meetings, and create paper copies for group discussions.\nAlternatively, your group may decide to project this template to a screen and do work on-line during meetings. This approach requires a cohesive group.\nAlternates: individuals assigned as back-ups to specific emergency response positions.\nEmergency Action Plan (EAP): prepared by each campus department/unit, the EAP is specific to each work site and outlines various emergency responsibilities of staff, evacuation routes, emergency Meeting areas, emergency supplies, emergency notification plans, etc.\nDepartment Team Leader: Designated by the Department/Unit Head, as the primary emergency contact for the work site; responsible for implementation and updating of the Emergency Action Plan: in concert with the floor and building marshals, assist with the safe evacuation of the unit staff.\nEmergency Operations Center (EOC): The EOC is where campus emergency management staff members are deployed to coor","cbCainU9HQtm6RVi","https://ap.wps.com/l/cbCainU9HQtm6RVi","docx",958501,23,"English","# Introduction\n## Purpose and scope\n## How to submit or update\n# Tips—Before you begin\n## Planning group and team leader\n# Information fields\n## Evacuation and supplies locations\n## Emergency response roles","[{\"question\":\"What is the purpose of a Department Emergency Action Plan (EAP)?\",\"answer\":\"It is a flexible planning tool that helps departments prepare and respond to emergencies, reduce risk and loss of life, and ensure faculty, staff, and students understand their worksite emergency actions.\"},{\"question\":\"Which details must a department fill in on the EAP template?\",\"answer\":\"The template collects unit and worksite evacuation protocols and emergency meeting locations, personnel assignments (including marshals), special needs and contact information, communication/notification procedures, and key supplies and equipment locations.\"},{\"question\":\"How often must the EAP be updated and submitted?\",\"answer\":\"The plan must be submitted for review and updated through annual updates, including routine review and testing of the Department EAP each year.\"}]","EMERGENCY ACTION PLAN - Department/College/Unit Preparedness and Response | DOCX",8]
