Overview  \nThe State of Delaware Department of Health and Social Services, Division of Developmental Disabilities Services (DDDS) , seeks professional services to provide dental care for persons with developmental disabilities who are DDDS service recipients living in a provider-managed residential setting. DDDS recognizes that oral health is an important component of overall health This request for proposals (“RFP”) is issued pursuant to 29 Del. C. §§ 6981 and 6982.  \nThe proposed schedule of events subject to the RFP is outlined below:  \nPublic Notice  \nDeadline for Questions  \nResponse to Questions Posted by: Deadline for Receipt of Proposals Estimated Notification of Award  \nDate: 04/02/2019  \nDate: 04/16/2019  \nDate: 04/30/2019  \nDate: 05/14/2019 at 11:00 AM (Local Time)  \nDate: 05/28/2019  \nEach proposal must be accompanied by a transmittal letter which briefly summarizes the proposing firm’s interest in providing the required professional services. The transmittal letter must also clearly state and justify any exceptions to the requirements of the RFP which the  \napplicant may have taken in presenting the proposal. (Applicant exceptions must also be recorded on Attachment 3) .  \nAdditionally Appendix “B” Template has been attached for proposal submission purposes. Dental Vendors interested in working with the Division of Developmental Disabilities must complete this form.  \nThis is an open and continuous RFP. Following this initial advertisement, the DIVISION will accept applications between March 1st and March 31st and September 1st through September 30th of each calendar year. Awarded Vendors will be notified within 30 days of the submission deadline.  \nThe State of Delaware reserves the right to deny any and all exceptions taken to the RFP requirements.  \nMANDATORY PREBID MEETING  \nA mandatory pre-bid meeting has not been established for this Request for Proposal.  \nII. Scope of Services  \nA. Description of Services:  \n1. DDDS may refer individuals living in a provider-managed DDDS residential setting to dental providers for dental screenings and routine cleanings or other dental services as specified by the DIVISION.  \n2. The Awarded Vendor will provide dental services consistent with applicable regulations, standards, and procedures governing the best practice of dentistry.  \n3. The Awarded Vendor determines the client’s dental status through dental examinations and diagnostic tests.  \n4. The Awarded Vendor will develop a dental treatment plan for each individual referred by the DIVISION based on industry-standard dental examinations and diagnostic tests. The treatment plan must include the following information for each recommended dental service: dental procedure code, procedure code description , tooth number, tooth surface, Medicaid fee charged and invoice number. All DDDS consumers served must have a dental treatment plan that is approved by DDDS before any dental work can be performed.  \na.
