Criteria\nThe children’s community occupational therapy service provides equipment on an assessed and needs led basis to children and young people at home.  The equipment provided is to enable occupations and participation in daily life. The equipment is provided through the Integrated Community Equipment Store.\nIn this document equipment refers to specialist disability equipment, which is not readily commercially available.  As part of the assessment process we may ask service users to trial commercially available options prior to the provision of more specialist items.\nEquipment is for use of individuals’ only, provided to their main residence. It would be expected that any safely transportable equipment would not be requested in duplicate. A case will need to be made for duplicate equipment where the child spends 50% of their time with either parent.\nThe hospital discharge team will provide the equipment for temporary use up to 16-weeks post discharge.\nA referral to Housing Occupational therapy team will need to be made for major adaptation and ceiling track hoists.\nThis documents also relates to be used for the provision of equipment that is funded by mainstream and special schools.\nThis document is a guideline for the commonly used equipment which is provided by the service.
