These individuals work one-on-one with participants to elicit motivation for change, provide education, connect participants with local resources, facilitate the SMART goal-setting process, and provide on-going support to help participants achieve their health goals. This innovative approach is central to achieving the positive health outcomes of hundreds of our participants. This toolkit is centered on our health coaching practice as an innovative practice in order to assist other medical settings with implementing a patient-centered approach to behavior change. \nBackground \n􀁸 Problem \n􀁸 What we did \n􀁸 Outcomes \n􀁸 Evaluation \nBackground \nProblem \nCatherine’s Health Center, a Federally Qualified Health Center Look Alike in Grand Rapids, Michigan, serves low-income, un-and under-insured individuals and families. People living in poverty face multiple complex barriers to improving or sustaining their health, including inadequate access to quality care, inability to navigate the healthcare system, poor health literacy, and many other social determinants of health. When patients don’t have the necessary resources and support, they experience higher rates of cardiovascular disease and other chronic diseases. The cost burden often falls on emergency departments and the broader community, while these individuals and families experience a powerful reduction in quality and years of life. \nWhat we did \nOur aim was to engage participants as active partners in their care rather than simply recipients of services in order to discover their needs, offer community-based solutions, and build an innovative evidence-based approach to improve heart health for the underserved. \nThrough generous funding from the AstraZeneca HealthCare Foundation’s Connections for Cardiovascular HealthSM program, we built Live Heart Smart. We based the program on Michigan’s WISEWOMAN ProgramSM which is part of the Michigan Department of Health and Human Services, funded by ","cbCaicdjf8rTOyPu","https://ap.wps.com/l/cbCaicdjf8rTOyPu","pdf",11306777,75,"English","# Purpose of Toolkit\n# Background\n## Problem\n## What we did\n## Outcomes\n## Evaluation\n# Theory\n## Transtheoretical model\n## Motivational Interviewing\n# Recruitment & Training\n## Recruitment\n## Training\n# Participant Recruitment & Health Coaching Process\n# Forms\n## Live Heart Smart Intake Survey\n## Readiness and Confidence to Change Ruler\n## Participant Agreement\n## Follow-up Contact Form\n## Outcome Evaluation Contact Form\n# References\n# Appendix A: Additional Resources\n# Appendix B: Health education","[{\"question\":\"What problem does the toolkit target?\",\"answer\":\"It targets barriers faced by people living in poverty, including limited access to quality care, difficulty navigating the healthcare system, and low health literacy, which contribute to higher cardiovascular and chronic disease rates.\"},{\"question\":\"How does the program support participants to change behavior?\",\"answer\":\"It uses one-on-one risk reduction counseling and SMART goal setting through motivational interviewing techniques, combined with education, community resource connection, and ongoing support.\"},{\"question\":\"What outcomes are reported for the participants?\",\"answer\":\"Between January 2014 and December 2017, screened participants showed improvements such as lower blood pressure and A1C, reduced LDL-C and triglycerides, increased HDL, reductions in total cholesterol and glucose, and weight and BMI decreases.\"}]","Catherine’s Health Center - Health Coaching Toolkit - 2018 | PDF",1791861421,26]
