A face-to-face hand-off communication that includes the patient (and even their family) is quickly becoming the best practice to prevent such errors. \nIn an article citing 33 studies, Bedside Shift Report has been shown to improve: \n• Patient satisfaction \n• Nurse satisfaction \n• Physician satisfaction \n• Patient safety \n• Accountability \n• Shift change carry over time \n• Support and mentoring of new nurses \n• Shift-to-shift relationships \n• Whiteboard use \n(Source: JONA Volume 44, Number 10, pp 541-545) \nOur experience has shown that Bedside Shift Report is indeed a great personal and peer accountability tool. You own the results of the care that you’ve delivered during your shift during this face-to-face report with the patient and the peer that is replacing you. \nOur personal experience with bedside report for both department to department as well as shift to shift is that both the nursing staff (as well as other members of the interdisciplinary care team) and patients felt this as truly a gift to share information and engage the patient. \nThat time when two nurses, or two teams collaborate to paint a picture of a surgery, or the last 12 hours is such a valuable demonstration of collaboration and teamwork, and patient and nurse engagement. \nWhy, if bedside handoffs are so darn good for the patient, physician, and nurse do they go undone in hospitals across the country on a daily basis? \nWhy don’t nurses simply get up from the desk and go to the patient’s room for report? \nWe have found that nurses report barriers to making an effective bedside handoff happen. \nAs far as barriers go regarding the implementation and ongoing success of bedside shift and interdepartmental handoffs, we have found in our experience the best way to train this best practice is to address the barriers immediately. \nCall the elephant out in the room. \nWhen nurses bring barriers to the forefront and actually create a plan to overcome barriers prior to implementation and kick off, everyone is aware, and can acknowledge potential issues. \nAll of the barriers of sleeping patients, HIPAA, and “I can’t find who I am supposed to be giving report to” can be managed prior to your kick-off or refresh of this valuable Solution. \nOne of the most frequent barriers commented on by nursing staff is a lack of time. It may initially appear to the nurses that going into a room to discuss the patients away from the bedside will indeed save time. Or, maybe your employees like to stand in the hallway to give the report and hand off. \n(Kind of like dipping your toes in the water and calling it swimming. ) \nTime is wasted finding other employees to give report to and may also be perceived as wasted when the patient decides to be chatty. Respond in your training to address lost employees and chatty patients. Make sure nurses know the key words to say in those situations. My experience has been that you save so much time on 90% of your patients, that the 10% who have to go to the bathroom in the middle of the report or want to discuss their life story will not hamper your efforts to make the hand off happen in a timely fashion. \nA huge time waster, however are those nurses who have been giving report in the same way for 30 years, and still insist that the oncoming shift needs to know what the patient’s abnormal labs were 3 weeks ago. We recommend an acronym (created and adopted by nurses) to standardize the b","cbCaikmAIlbLseUN","https://ap.wps.com/l/cbCaikmAIlbLseUN","pdf",316412,"English","# Bedside Shift Report: Purpose and Impact\n## Evidence of benefits and patient outcomes\n## Accountability through face-to-face handoffs\n# Barriers to Implementation and Training Approach\n## Identifying and addressing barriers before go-live\n## Managing time issues and patient engagement\n# Standardizing Handoffs with Acronyms\n## IPASS framework for bedside communication\n## Reducing communication errors and improving teamwork\n# Validation and Ongoing Improvement\n## Data, leader rounding, competency labs, and patient feedback","[{\"question\":\"Why does the document emphasize bedside shift and interdepartmental handoffs?\",\"answer\":\"Bedside handoffs improve communication and engage patients and families, helping prevent serious preventable adverse events.
